Description
Overview of the medicine
Amzith 200 mg Suspension is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted infections.
Uses & Indications
Dosage
Adults
Not typically used for adults in suspension form; adults usually take tablets/capsules.
Elderly
No specific dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment (creatinine clearance > 10 mL/min). Exercise caution in severe impairment.
How to Take
Administer orally once daily, with or without food. Measure the dose accurately using the provided measuring device. Shake well before use.
Mechanism of Action
Azithromycin works by inhibiting bacterial protein synthesis. It binds to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translation of mRNA and ultimately inhibiting bacterial growth.
Pharmacokinetics
Onset
Within 2-3 hours (clinical effect may take longer to manifest)
Excretion
Mainly excreted unchanged in bile (approximately 50%), with a smaller amount (about 6%) excreted in urine.
Half life
Approximately 2-4 days (due to extensive tissue uptake and slow release)
Absorption
Rapidly absorbed from the gastrointestinal tract; however, bioavailability is approximately 37%. Peak plasma concentrations occur 2-3 hours after administration. Food can reduce peak concentrations but total absorption remains similar.
Metabolism
Primarily hepatic, undergoes N-demethylation; however, a significant portion is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum digoxin levels; monitor digoxin levels.
Antacids
Reduced peak plasma levels of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Potentiates anticoagulant effect; monitor PT/INR closely.
Nelfinavir
Increased azithromycin concentrations; monitor for known azithromycin side effects.
Cyclosporine
Increased cyclosporine levels; use with caution and monitor cyclosporine concentrations.
Ergot alkaloids
Potential for ergotism (vasospasm and dysesthesia) due to severe vasoconstriction; concomitant use is contraindicated.
Storage
Store powder below 30°C in a dry place, away from light. After reconstitution, store at room temperature (20-25°C) or refrigerated (2-8°C) as per specific product instructions, for the recommended duration.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures as clinically indicated. Gastric lavage and supportive therapy are recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Azithromycin is excreted in human milk; caution should be exercised when administered to a nursing woman.
