Description
Overview of the medicine
Ancor-Plus 25 mg tablet is a combination lipid-lowering medication containing Atorvastatin (an HMG-CoA reductase inhibitor) and Ezetimibe (a cholesterol absorption inhibitor). It is used to reduce high cholesterol levels (LDL-C, total cholesterol, triglycerides) and increase HDL-C in patients with primary hypercholesterolemia or mixed dyslipidemia, especially when monotherapy is insufficient.
Uses & Indications
Dosage
Adults
Usually 1 tablet once daily, preferably in the evening, with or without food. Dosage should be individualized based on LDL-C levels, treatment goal, and patient response.
Elderly
No dosage adjustment required based on age alone. Caution advised due to potential for increased adverse effects.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. For severe renal impairment (CrCl <30 mL/min), consult a physician and consider lower starting doses.
How to Take
Oral administration. Can be taken with or without food, preferably in the evening.
Mechanism of Action
Atorvastatin competitively inhibits HMG-CoA reductase, an enzyme involved in cholesterol synthesis in the liver, leading to a reduction in LDL-C. Ezetimibe selectively inhibits the absorption of cholesterol and related phytosterols in the small intestine, thereby reducing the delivery of intestinal cholesterol to the liver.
Pharmacokinetics
Onset
Lipid-lowering effects observed within 2 weeks, maximal effect usually within 4 weeks.
Excretion
Atorvastatin: Primarily excreted in bile/feces. Ezetimibe: Primarily excreted in feces (major) and urine (minor).
Half life
Atorvastatin: ~14 hours (active metabolites contribute to 20-30 hours). Ezetimibe: ~22 hours (for total ezetimibe).
Absorption
Atorvastatin: Rapidly absorbed, peak plasma concentrations within 1-2 hours. Ezetimibe: Rapidly absorbed, peak plasma concentrations within 4-12 hours.
Metabolism
Atorvastatin: Primarily metabolized by CYP3A4 to active and inactive metabolites. Ezetimibe: Primarily metabolized in the small intestine and liver via glucuronide conjugation.
Side Effects
Contraindications
- •Hypersensitivity to Atorvastatin, Ezetimibe, or any component of the tablet.
- •Active liver disease or unexplained persistent elevations of serum transaminases.
- •Pregnancy and lactation.
Drug Interactions
Warfarin
Potential for increased INR, monitor closely.
Colchicine
Risk of myopathy/rhabdomyolysis with statins.
Cyclosporine
Increased plasma concentrations of ezetimibe and atorvastatin. Contraindicated.
Oral Contraceptives
Increased AUC of ethinyl estradiol and norethindrone.
Gemfibrozil / Other Fibrates
Increased risk of myopathy/rhabdomyolysis.
Strong CYP3A4 Inhibitors (e.g., Itraconazole, Ketoconazole, Erythromycin, Clarithromycin, HIV protease inhibitors)
Increased atorvastatin plasma concentrations, increased risk of myopathy.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. Treatment should be symptomatic and supportive. Due to high protein binding and extensive metabolism, hemodialysis is unlikely to significantly enhance atorvastatin or ezetimibe clearance.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Women of childbearing potential should use effective contraception. If pregnancy occurs, discontinue immediately.
