Overview of the medicine
Losartan is an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure, reduce the risk of stroke in patients with high blood pressure and an enlarged heart, and to treat kidney disease in patients with type 2 diabetes.
Uses & Indications
Dosage
Adults
Initial dose is usually 50 mg once daily. Dosage may be adjusted from 25 mg to 100 mg once daily based on blood pressure response. For 25 mg, it may be used as a starting dose for certain patients or for specific indications.
Elderly
No initial dosage adjustment required. However, consider a lower starting dose (e.g., 25 mg) for elderly patients, especially those with potential volume depletion or hepatic impairment.
Renal_impairment
No initial dosage adjustment required for mild to moderate renal impairment. For severe renal impairment or patients on dialysis, a lower starting dose (e.g., 25 mg) may be considered.
How to Take
Angilock-25 mg tablet should be taken orally once daily, with or without food. It is important to take it at the same time each day.
Mechanism of Action
Losartan works by blocking the binding of angiotensin II to its AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This prevents vasoconstriction and aldosterone secretion, leading to reduced blood pressure.
Pharmacokinetics
Onset
Within 1 hour, peak effect at 6 hours.
Excretion
Approximately 35% excreted in urine and 60% in feces, as unchanged drug and metabolites.
Half life
Losartan: 2 hours; Active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed orally, undergoes significant first-pass metabolism. Bioavailability is approximately 33%.
Metabolism
Extensive hepatic metabolism, primarily by CYP2C9 and CYP3A4 to an active carboxylic acid metabolite (E-3174).
Side Effects
Contraindications
- •Hypersensitivity to Losartan or any component of the formulation
- •Pregnancy (especially second and third trimesters)
- •Concomitant use with Aliskiren in patients with diabetes
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal impairment.
Potassium-sparing diuretics/Potassium supplements
May lead to hyperkalemia (high potassium levels).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) could occur from parasympathetic stimulation. Treatment is symptomatic and supportive, including IV fluids and vasopressors if severe hypotension occurs.
Pregnancy & Lactation
Contraindicated in the second and third trimesters of pregnancy due to fetal harm. Not recommended during breastfeeding as it may pass into breast milk.
