Overview of the medicine
Angilock 50 mg Tablet contains Losartan Potassium, which is an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure (hypertension), to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy, and to treat kidney disease in patients with type 2 diabetes and a history of high blood pressure.
Uses & Indications
Dosage
Adults
Initial dose is usually 50 mg once daily. Dosage can be increased to 100 mg once daily if needed. For diabetic nephropathy, initial dose is 50 mg once daily, adjusted based on blood pressure response up to 100 mg once daily.
Elderly
No initial dosage adjustment is generally required, but close monitoring is advised.
Renal_impairment
No initial dosage adjustment is generally required for patients with mild to moderate renal impairment. For severe renal impairment, lower initial dose (e.g., 25 mg) may be considered.
How to Take
Angilock 50 mg Tablet should be taken orally, with or without food, usually once daily. It should be swallowed whole with water.
Mechanism of Action
Losartan works by blocking the binding of angiotensin II to the AT1 receptor, primarily found in vascular smooth muscle and the adrenal gland. This action inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation, reduced sodium and water retention, and ultimately lower blood pressure.

Approximately 35% excreted in urine and 60% in feces (biliary excretion) as unchanged drug and metabolites.

Losartan: 2 hours; Active metabolite (EXP3174): 6-9 hours
Absorption
Rapidly absorbed from the gastrointestinal tract; undergoes significant first-pass metabolism. Bioavailability is about 33%.
Metabolism
Extensively metabolized by CYP2C9 and CYP3A4 to an active carboxylic acid metabolite (EXP3174) and several inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Losartan or any component of the tablet
- •Pregnancy (second and third trimesters)
- •Concomitant use with aliskiren in patients with diabetes
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity have been reported.
Diuretics and other antihypertensive agents
May enhance the hypotensive effect.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Losartan and increase the risk of renal impairment.
Potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium
May lead to hyperkalemia.
Storage
Store in a cool and dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure) and tachycardia (rapid heartbeat); bradycardia (slow heartbeat) may occur. Management involves symptomatic and supportive treatment, including intravenous fluid administration for hypotension.
Pregnancy & Lactation
Pregnancy: Contraindicated in the second and third trimesters due to fetal toxicity. Discontinue as soon as pregnancy is detected. Lactation: Not recommended during breastfeeding as Losartan and its active metabolite may be excreted in human milk.
