Overview of the medicine
Antanil (Esomeprazole) is a proton pump inhibitor used to treat conditions caused by excess stomach acid, such as GERD, peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
GERD: 20-40 mg once daily for 4-8 weeks. Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. H. pylori eradication: 20 mg twice daily with appropriate antibiotics for 7-10 days.
Elderly
No specific dosage adjustment is usually required, but caution is advised in patients with severe hepatic impairment.
Renal_impairment
No dosage adjustment is needed for patients with renal impairment.
How to Take
Take Antanil capsules at least one hour before a meal. Swallow the capsule whole with water. Do not chew, crush, or open the capsule.
Mechanism of Action
Esomeprazole selectively inhibits the H+/K+-ATPase enzyme system (proton pump) located at the secretory surface of the gastric parietal cells, thereby blocking the final step of acid production.

Excretion
Mainly via urine (about 80%) and feces (about 20%) as inactive metabolites.
Half life
Approximately 1-1.5 hours
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur in about 1-2 hours. Bioavailability is approximately 89% after repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.
Drug Interactions
Digoxin
Increased systemic exposure to digoxin.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate plasma levels, potentially leading to toxicity.
Atazanavir/Nelfinavir
Reduced plasma concentrations of these antiretrovirals.

Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
