Description
Overview of the medicine
Aripiprazole is an atypical antipsychotic medication used to treat mental health conditions such as schizophrenia, bipolar I disorder (manic or mixed episodes), and as an add-on treatment for major depressive disorder. It helps to restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
For Schizophrenia: Initial 10-15 mg once daily, target dose 10-30 mg/day. For Bipolar I Disorder: Initial 15 mg once daily, target dose 15-30 mg/day. For MDD (adjunct): Initial 2-5 mg once daily, target dose 5-15 mg/day.
Elderly
No specific dose adjustment is generally required, but caution is advised. Start with lower doses and titrate slowly.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Aripiprazole is thought to work by partially agonising dopamine D2 and serotonin 5-HT1A receptors, and antagonising serotonin 5-HT2A receptors. This unique combination helps stabilize dopamine and serotonin systems, improving symptoms of psychosis and mood disorders.

Side Effects
Contraindications

Drug Interactions
CYP3A4 inducers (e.g., carbamazepine, rifampin)
May decrease aripiprazole plasma levels. Dose increase of aripiprazole may be necessary.
CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
May increase aripiprazole plasma levels. Dose reduction of aripiprazole may be necessary.
Other CNS depressants (e.g., alcohol, benzodiazepines)
Increased risk of sedation and CNS depression.
CYP2D6 inhibitors (e.g., fluoxetine, paroxetine, quinidine)
May increase aripiprazole plasma levels. Dose reduction of aripiprazole may be necessary.

Overdose
Symptoms of overdose include drowsiness, vomiting, tremor, tachycardia, agitation, aggression, hypotension, extrapyramidal symptoms, and seizures. Management is primarily supportive, including maintaining an adequate airway, oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Neonates exposed to antipsychotic drugs during the third trimester of pregnancy are at risk for extrapyramidal and/or withdrawal symptoms. Aripiprazole is excreted in human breast milk; a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
