Description
Overview of the medicine
Aprodust 4 mg Tablet contains Aprixin, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder, anxiety disorders, and panic disorder. It works by increasing the levels of serotonin in the brain, which helps to improve mood and reduce symptoms of depression and anxiety.
Uses & Indications
Dosage
Adults
Initial dose 4 mg once daily, may be increased to 8-16 mg daily based on response. Max 20 mg/day.
Elderly
Lower initial dose (e.g., 2 mg once daily) and slower titration may be considered due to potential for increased sensitivity.
Renal_impairment
No dose adjustment generally required for mild to moderate impairment; use with caution in severe impairment.
How to Take
Take orally once daily, with or without food, preferably at the same time each day. Do not crush or chew the tablet.
Mechanism of Action
Aprixin selectively inhibits the reuptake of serotonin (5-HT) into presynaptic neurons, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission in the central nervous system, which is believed to be responsible for its antidepressant and anxiolytic effects.
Pharmacokinetics
Onset
Initial effects visible in 1-2 weeks; full therapeutic effects may take 4-6 weeks.
Excretion
Excreted mainly through urine (renal) and feces (biliary).
Half life
Approximately 20-30 hours.
Absorption
Well absorbed orally; peak plasma concentrations reached in 4-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to Aprixin or any component of the tablet
- •Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing MAOI therapy
- •Concomitant use with pimozide or thioridazine
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
Warfarin
Increased risk of bleeding due to potential inhibition of CYP2C19, affecting warfarin metabolism.
Tricyclic Antidepressants (TCAs)
Increased plasma concentrations of TCAs, potentially leading to increased adverse effects.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include drowsiness, nausea, vomiting, tachycardia, tremor, agitation, and rarely seizures. Management is supportive, including maintaining an airway, monitoring cardiac and vital signs. Activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as Aprixin is excreted in breast milk.
