Overview of the medicine
Apulset 4 mg Oral Solution contains Levocetirizine, an antihistamine used to relieve symptoms of allergies such as hay fever and chronic urticaria. It works by blocking a natural substance (histamine) that your body makes during an allergic reaction.
Uses & Indications
Dosage
Adults
The usual recommended dose for adults and adolescents 12 years and older is 5 mg (e.g., as prescribed based on the concentration of this 4mg solution) once daily in the evening. For this 4 mg oral solution, follow physician's instructions, often 4 mg once daily.
Elderly
No dose adjustment is generally required for elderly patients with normal renal function. For elderly patients with impaired renal function, dosage should be adjusted based on creatinine clearance.
Renal_impairment
For moderate renal impairment (CrCl 30-49 mL/min), the recommended dose is 5 mg (or 4 mg if this is the prescribed unit) every other day. For severe renal impairment (CrCl 10-29 mL/min), 5 mg (or 4 mg) twice weekly. Contraindicated in end-stage renal disease.
How to Take
Apulset oral solution should be taken orally, with or without food. It is generally recommended to take the dose in the evening. Use the provided measuring device to ensure accurate dosing.
Mechanism of Action
Levocetirizine is a potent and highly selective H1-receptor antagonist. It blocks the binding of histamine to H1 receptors, thereby preventing the histamine-mediated effects like vasodilation, increased vascular permeability, itching, and bronchoconstriction associated with allergic reactions.
Pharmacokinetics
Onset
Onset of action within 15-30 minutes.
Excretion
Primarily excreted unchanged in urine (85.4% of dose) and feces (12.9% of dose). Renal excretion is the major route of elimination.
Half life
Approximately 7-10 hours in healthy adults; prolonged in patients with renal impairment.
Absorption
Rapid and extensive absorption after oral administration, peak plasma concentrations reached in approximately 1 hour. Food does not affect the extent of absorption but may slightly decrease the rate.
Metabolism
Less than 14% of the dose is metabolized, primarily by aromatic oxidation, N-dealkylation, O-dealkylation, and taurine conjugation. Metabolic pathways are not fully elucidated.
Side Effects
Contraindications
- •Hypersensitivity to levocetirizine, cetirizine, hydroxyzine, or any other piperazine derivatives, or to any of the excipients.
- •Patients with severe renal impairment (creatinine clearance less than 10 mL/min).
- •Patients undergoing hemodialysis.
Drug Interactions
Ritonavir
May increase plasma concentrations of levocetirizine; however, the clinical significance is generally minor.
Theophylline
May slightly decrease the clearance of levocetirizine, but no significant interaction requiring dose adjustment has been observed.
Alcohol & CNS Depressants
Concurrent use with alcohol or other CNS depressants may increase CNS depression and impair alertness.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children. Do not freeze.
Overdose
Symptoms of overdose may include drowsiness in adults and initial agitation and restlessness followed by drowsiness in children. In case of overdose, symptomatic or supportive treatment should be initiated as there is no specific antidote. Gastric lavage may be considered soon after ingestion.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Levocetirizine is excreted in human breast milk; therefore, caution should be exercised when administered to a nursing woman.
