Overview of the medicine
Apulset 8 mg Tablet contains Apulsartan, an Angiotensin Receptor Blocker (ARB), used primarily for the treatment of high blood pressure (hypertension) and heart failure. It helps to relax blood vessels, lowering blood pressure and making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
For hypertension, the usual initial dose is 8 mg once daily. The dose may be increased to 16 mg or 32 mg once daily depending on the blood pressure response. For heart failure, the initial dose is typically 4 mg once daily, titrated up to 16 mg or 32 mg once daily.
Elderly
No specific dose adjustment is required for elderly patients, but careful monitoring is advised.
Renal_impairment
For mild to moderate renal impairment, no initial dose adjustment is necessary. For severe renal impairment (CrCl < 30 mL/min), a lower initial dose of 4 mg once daily may be considered.
How to Take
Apulset tablets should be swallowed whole with water, with or without food. It is recommended to take the tablet at the same time each day.
Mechanism of Action
Apulsartan selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased blood pressure, and reduced fluid retention.

Onset
Antihypertensive effects usually within 2 hours; maximal effect within 2-4 weeks.
Excretion
Primarily excreted via feces (60-70%) and urine (30-40%), both as unchanged drug and metabolites.
Half life
Approximately 6-9 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract; absolute bioavailability is approximately 25-30%. Peak plasma concentrations are reached within 2-4 hours.
Metabolism
Primarily metabolized by CYP2C9 and UGT enzymes, forming an active metabolite (M1) and inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Apulsartan or any component of the formulation
- •Pregnancy (especially second and third trimesters)
- •Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR < 60 mL/min/1.73 m2)
- •Severe hepatic impairment
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity. Monitor lithium levels carefully.
Aliskiren
Contraindicated in patients with diabetes or renal impairment due to increased risk of hypotension, hyperkalemia, and renal impairment.
Diuretics (Potassium-sparing)
Increased risk of hyperkalemia.
NSAIDs (Non-steroidal anti-inflammatory drugs)
May attenuate the antihypertensive effect and increase the risk of renal impairment, especially in dehydrated or elderly patients.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
The most likely manifestation of overdose is hypotension and tachycardia; bradycardia could also occur. Symptomatic treatment and supportive measures should be initiated. Apulsartan is not effectively removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category D. Apulsartan is contraindicated during pregnancy, especially during the second and third trimesters, due to risks of fetal injury and death. It is not known whether Apulsartan is excreted in human milk; caution should be exercised when administered to a nursing woman.
