Description
Overview of the medicine
Aquacal-D is a combination of Calcium Carbonate and Vitamin D3, used to treat and prevent calcium and vitamin D deficiencies, and to support bone health.
Uses & Indications
Dosage
Adults
Typically 1-2 tablets daily, preferably with or after meals. Dosage may vary based on individual needs and physician's advice.
Elderly
Similar to adult dosage, but caution should be exercised in patients with impaired renal function. Consult a physician.
Renal_impairment
Use with caution. Dosage adjustments may be necessary, and regular monitoring of serum calcium and phosphate levels is recommended. Not recommended in severe renal impairment.
How to Take
Take the tablet orally with a glass of water, preferably with or immediately after a meal to enhance absorption and minimize gastrointestinal upset. Do not chew or crush.
Mechanism of Action
Calcium Carbonate provides elemental calcium essential for bone formation and maintenance. Vitamin D3 (Cholecalciferol) helps in the absorption of calcium from the gut and its deposition into the bones.

Onset
Effects on bone mineralization are gradual and may take several weeks or months.
Excretion
Calcium is excreted via urine and feces. Vitamin D3 metabolites are primarily excreted in bile and feces.
Half life
Elemental calcium has complex kinetics; Vitamin D3's active metabolites have a half-life ranging from days to weeks.
Absorption
Calcium carbonate is absorbed in the gastrointestinal tract, primarily in the small intestine. Vitamin D3 is absorbed from the small intestine (requires bile salts) and then hydroxylated in the liver and kidneys to its active forms.
Metabolism
Vitamin D3 undergoes hydroxylation in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to 1,25-dihydroxyvitamin D (calcitriol), the active form.
Side Effects
Contraindications

- •Hypercalcemia (high calcium levels in blood)
- •Hypercalciuria (high calcium levels in urine)
- •Severe renal impairment
- •Hypervitaminosis D (excess vitamin D)
Drug Interactions
Levothyroxine
Calcium may impair absorption. Separate administration by at least 4 hours.
Bisphosphonates
Reduced absorption. Administer calcium at a different time.
Corticosteroids
May reduce calcium absorption and vitamin D effects.
Cardiac glycosides
Hypercalcemia increases the risk of cardiac arrhythmias. Close monitoring is needed.
Thiazide diuretics
May increase the risk of hypercalcemia by reducing calcium excretion.
Tetracyclines and Quinolone antibiotics
Calcium can chelate these antibiotics, reducing their absorption. Administer at least 2 hours before or 4-6 hours after calcium supplements.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose (hypercalcemia) include nausea, vomiting, constipation, abdominal pain, muscle weakness, fatigue, polydipsia, polyuria, anorexia, and in severe cases, cardiac arrhythmias and coma. Management involves immediate discontinuation of the supplement, ensuring adequate hydration, and symptomatic treatment. In severe cases, hospitalization may be required.
Pregnancy & Lactation
Aquacal-D is generally considered safe and often recommended during pregnancy and lactation to meet the increased calcium and vitamin D requirements. However, doses should not exceed the recommended daily allowance, and it should be used under medical supervision.
