Description
Overview of the medicine
Ardance-L 25 mg Tablet contains Losartan, an Angiotensin II Receptor Blocker (ARB) used to treat high blood pressure, diabetic nephropathy, and heart failure. It helps relax blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Hypertension: Initial dose of 50 mg once daily. Dose can be increased to 100 mg once daily. Diabetic Nephropathy: Initial dose of 50 mg once daily, may be increased to 100 mg once daily. Heart Failure: Initial dose of 12.5 mg once daily, gradually titrated up to 50 mg once daily.
Elderly
No initial dosage adjustment is typically needed for elderly patients, but monitoring of renal function and potassium levels is recommended.
Renal_impairment
No initial dosage adjustment is needed for mild to moderate renal impairment. For severe renal impairment or patients on dialysis, a lower initial dose of 25 mg once daily may be considered. Monitor renal function closely.
How to Take
Take Ardance-L tablet orally once daily with or without food. Swallow the tablet whole with water.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues (e.g., vascular smooth muscle, adrenal gland, kidney). This blockage prevents the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to decreased blood pressure and reduced workload on the heart.
Pharmacokinetics
Onset
Within 6 hours (antihypertensive effect), full effect may take 3-6 weeks.
Excretion
Approximately 35% excreted in urine and 60% in feces (biliary excretion), primarily as unchanged drug and active metabolite.
Half life
Losartan: about 2 hours; active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed from the gastrointestinal tract; bioavailability is approximately 33%. Peak plasma concentrations occur about 1 hour after oral administration for Losartan and 3-4 hours for its active metabolite.
Metabolism
Undergoes significant first-pass metabolism by CYP2C9 and CYP3A4 enzymes to an active carboxylic acid metabolite (E-3174) and several inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Losartan or any component of the formulation.
- •Pregnancy (especially during the second and third trimesters).
- •Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR <60 mL/min/1.73 m²).
- •Severe hepatic impairment.
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Losartan and increase the risk of renal function deterioration, especially in elderly, volume-depleted, or renally impaired patients.
Dual blockade of the Renin-Angiotensin System (RAS) with ACE inhibitors or aliskiren
Increased risk of hypotension, hyperkalemia, and renal impairment; generally not recommended.
Potassium-sparing diuretics (e.g., spironolactone, amiloride, triamterene) or potassium supplements
May lead to increased serum potassium levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension (low blood pressure), tachycardia (rapid heartbeat), and possibly bradycardia (slow heartbeat). Management involves symptomatic and supportive treatment, including intravenous fluids for hypotension.
Pregnancy & Lactation
Pregnancy: Contraindicated during the second and third trimesters due to fetal toxicity. Discontinue as soon as pregnancy is detected. Lactation: Not recommended during breastfeeding as it is unknown if Losartan is excreted in human milk.
