Description
Overview of the medicine
Aripiprazole is an atypical antipsychotic medication primarily used to treat schizophrenia, bipolar I disorder, and as an add-on treatment for depression. It helps to restore the balance of certain natural substances (neurotransmitters) in the brain. Aride 50 mg tablet is a higher strength formulation for specific patient needs or maintenance therapy.
Uses & Indications
Dosage
Adults
For Schizophrenia, typical target dose is 10-30 mg once daily. For Bipolar Mania, 15 mg once daily as monotherapy. For adjunctive MDD, 2-15 mg once daily. A 50 mg dose may be used in some resistant cases or specific regimens as determined by a physician, but is generally higher than initial recommended doses.
Elderly
Lower starting doses are recommended (e.g., 5-10 mg/day) with slow titration, monitoring for increased sensitivity to side effects.
Renal_impairment
No dosage adjustment is generally required.
How to Take
Take Aride 50 mg tablet orally once daily, with or without food. Swallow the tablet whole with water; do not crush, chew, or break it.
Mechanism of Action
Aripiprazole's mechanism of action is thought to be due to partial agonism at D2 and 5-HT1A receptors and antagonism at 5-HT2A receptors. This unique pharmacological profile contributes to its mood-stabilizing and antipsychotic effects with a lower propensity for certain side effects compared to traditional antipsychotics.
Pharmacokinetics
Onset
Therapeutic effects may take several days to weeks to fully manifest. Acute agitation may respond faster with injectables.
Excretion
Excreted via feces (55%) and urine (25%).
Half life
Aripiprazole has an elimination half-life of approximately 75 hours, and its active metabolite, dehydro-aripiprazole, has a half-life of about 94 hours.
Absorption
Well absorbed orally, peak plasma concentrations typically within 3-5 hours. Absolute bioavailability is 87%.
Metabolism
Primarily metabolized by the liver enzymes CYP2D6 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to aripiprazole or any component of the formulation
Drug Interactions
CNS Depressants (e.g., alcohol, benzodiazepines)
Concomitant use may potentiate CNS depressive effects.
CYP3A4 Inducers (e.g., carbamazepine, rifampin, phenytoin)
May decrease aripiprazole levels; dose increase of aripiprazole may be needed.
CYP2D6 Inhibitors (e.g., fluoxetine, paroxetine, quinidine)
May increase aripiprazole levels; dose reduction of aripiprazole may be needed.
CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole, clarithromycin)
May increase aripiprazole levels; dose reduction of aripiprazole may be needed.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include somnolence, vomiting, tachycardia, and potentially extrapyramidal symptoms or seizures. Management should be supportive, including airway management, oxygenation, ventilation, and cardiovascular monitoring. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Neonates exposed to antipsychotic drugs during the third trimester of pregnancy are at risk for extrapyramidal and/or withdrawal symptoms. Aripiprazole is excreted in human milk; advise women not to breastfeed during treatment.
