Description
Overview of the medicine
Arinet (Desloratadine) is a non-drowsy antihistamine used to relieve symptoms of allergic rhinitis and chronic idiopathic urticaria. It provides effective relief from sneezing, runny nose, itchy nose or eyes, congestion, and skin itching and hives.
Uses & Indications
Dosage
Adults
5 mg orally once daily, with or without food.
Elderly
No dose adjustment generally required, but caution is advised in elderly patients with renal or hepatic impairment.
Renal_impairment
For patients with severe renal impairment (creatinine clearance < 30 mL/min), 5 mg every other day is recommended.
Hepatic_impairment
For patients with hepatic impairment, 5 mg every other day is recommended.
How to Take
Take Arinet orally with a glass of water, with or without food. Do not crush or chew the tablet; swallow it whole.
Mechanism of Action
Desloratadine is a potent, selective, peripheral H1-receptor antagonist. It blocks the action of histamine, a substance in the body that causes allergic symptoms. It also exhibits anti-inflammatory effects by inhibiting the release of pro-inflammatory mediators from mast cells and basophils.

Onset of action within 1 hour.
Excretion
Primarily excreted via urine (41%) and feces (47%) as metabolites.
Half life
Terminal elimination half-life is approximately 27 hours.
Absorption
Rapidly absorbed after oral administration; peak plasma concentrations reached in approximately 3 hours.
Metabolism
Extensively metabolized to 3-hydroxydesloratadine, an active metabolite, via hydroxylation followed by glucuronidation. CYP2D6 is involved in metabolism.

Desloratadine is excreted into breast milk; therefore, a decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
