Overview of the medicine
Aronem 1 gm Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat a wide range of serious bacterial infections, including those resistant to other antibiotics. It is administered intravenously.
Uses & Indications
Dosage
Adults
Usually 500 mg to 1 gm every 8 hours by intravenous infusion over 15-30 minutes, or intravenous bolus over 3-5 minutes, depending on the type and severity of infection.
Elderly
Dosage adjustment may be necessary based on renal function, similar to adults with renal impairment.
Renal_impairment
Significant dose reduction required based on creatinine clearance. For CrCl 26-50 mL/min: 1 gm every 12 hours. For CrCl 10-25 mL/min: 500 mg every 12 hours. For CrCl <10 mL/min: 500 mg every 24 hours.
How to Take
Administer by intravenous infusion over 15-30 minutes or by intravenous bolus over 3-5 minutes. Reconstitute with sterile water for injection, sodium chloride injection, or dextrose injection as per manufacturer's instructions.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Primarily via the kidneys, with about 70% excreted unchanged in urine.

Side Effects
Contraindications
- •Known hypersensitivity to meropenem or any other carbapenem antibiotics.
- •Severe hypersensitivity (e.g., anaphylactic reaction, severe skin reaction) to any other type of beta-lactam antibacterial agents (e.g., penicillins or cephalosporins).
Drug Interactions
Probenecid
Concomitant administration with probenecid inhibits the renal excretion of meropenem, leading to increased plasma concentrations and prolonged half-life of meropenem.
Valproic Acid
Meropenem may reduce valproic acid levels below the therapeutic range, leading to loss of seizure control. Co-administration is generally not recommended.
Oral Anticoagulants
Antibiotics, including meropenem, may enhance the anticoagulant effects of vitamin K antagonists like warfarin.
Storage
Store below 25°C, protected from light and moisture. Do not freeze. Reconstituted solutions should be used immediately or stored as per manufacturer's guidelines.
Overdose
In case of an overdose, symptomatic and supportive treatment should be given. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Meropenem should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk in very small quantities; caution should be exercised when administered to a nursing mother. Consult a doctor.
