Description
Overview of the medicine
Arotide 25 mcg Inhaler contains Fluticasone Propionate, an inhaled corticosteroid used for the prophylactic management of asthma and other chronic obstructive airway diseases. It reduces inflammation in the airways.
Uses & Indications
Dosage
Adults
Asthma: Initially 50-100 mcg twice daily. May be increased to 100-250 mcg twice daily depending on response. Maximum usually 500 mcg twice daily. COPD (off-label for monotherapy): Often 250-500 mcg twice daily.
Elderly
No specific dosage adjustment is generally required, but close monitoring is advised.
Renal_impairment
No dosage adjustment required.
How to Take
For oral inhalation only. Shake the inhaler well before use. Exhale fully, place mouthpiece in mouth, press canister while inhaling slowly and deeply. Hold breath for 10 seconds. Exhale slowly. Rinse mouth with water after use to prevent oral candidiasis.
Mechanism of Action
Fluticasone Propionate is a synthetic trifluorinated corticosteroid with potent anti-inflammatory activity. It works by reducing inflammation in the airways, thereby decreasing bronchial hyperresponsiveness and the frequency and severity of asthma symptoms. It achieves this by inhibiting the release of inflammatory mediators and altering gene expression related to inflammatory proteins.

Therapeutic effects typically begin within 24 hours, but full benefits may take 1-2 weeks or longer.
Excretion
Mainly excreted in the feces as metabolites, with some renal excretion.

Approximately 7.8 hours (terminal half-life after oral inhalation).
Absorption
Systemic absorption from the lungs is low; a significant portion is swallowed and undergoes extensive first-pass metabolism in the liver. Bioavailability is less than 1-2%.

Extensive hepatic metabolism, primarily by CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- •Acute bronchospasm or status asthmaticus (Fluticasone is not a rescue inhaler).
- •Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
- •Hypersensitivity to fluticasone propionate or any excipient.
Drug Interactions
Other Corticosteroids
Increased risk of systemic corticosteroid side effects if used concurrently (e.g., oral corticosteroids).
CYP3A4 Inhibitors (e.g., Ritonavir, Ketoconazole)
May significantly increase systemic exposure to fluticasone, leading to potential corticosteroid side effects. Co-administration is generally not recommended unless the potential benefit outweighs the risk of systemic corticosteroid side effects.
Storage
Store below 30°C. Protect from direct sunlight, heat, and freezing. Do not puncture or incinerate the canister.
Overdose
Acute overdose is unlikely to cause severe systemic effects. Chronic overdose may lead to signs of hypercorticism, including Cushing's syndrome, and adrenal suppression. Treatment is supportive; reduce dosage gradually.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown if fluticasone is excreted in human milk; use with caution in nursing mothers.
