Overview of the medicine
Arotide 50 mcg Inhalation Capsule contains Fluticasone Propionate, an inhaled corticosteroid used for the regular maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It helps to reduce inflammation in the airways.
Uses & Indications
Dosage
Adults
For Asthma: Initially 100-250 mcg twice daily. May be adjusted based on response up to 500-1000 mcg twice daily in severe cases. For COPD: 500 mcg twice daily.
Elderly
No specific dose adjustment is generally required, but close monitoring is advisable.
Renal_impairment
No specific dose adjustment is required.
How to Take
For oral inhalation only. The capsule should be placed in a suitable inhaler device (e.g., Rotahaler, Revolizer) and the contents inhaled through the mouth. Do not swallow the capsule. Rinse mouth with water after use to prevent oral candidiasis.
Mechanism of Action
Fluticasone Propionate is a synthetic trifluorinated corticosteroid that possesses potent anti-inflammatory activity. It acts by binding to glucocorticoid receptors in the airways, inhibiting the release of inflammatory mediators (e.g., cytokines, chemokines, eicosanoids) and reducing the activity of inflammatory cells (e.g., eosinophils, mast cells, lymphocytes). This reduces airway hyperresponsiveness and improves lung function.

Onset
Full therapeutic effect may take 4-7 days or longer, although some improvement may be seen within 24 hours.
Excretion
Primarily excreted in feces as metabolites; approximately 5% excreted in urine.

Approximately 8 hours (terminal half-life after IV administration).
Absorption
Low systemic absorption following inhalation; approximately 10-30% of the dose reaches the lungs, with the rest swallowed and undergoing extensive first-pass metabolism.

Extensive hepatic metabolism via CYP3A4 enzyme to inactive metabolites.
Side Effects
Contraindications
- •Acute bronchospasm or status asthmaticus (Fluticasone is not a rescue inhaler)
- •Known hypersensitivity to fluticasone propionate or any component of the formulation.
- •Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
Drug Interactions
Other corticosteroids
Increased risk of systemic corticosteroid effects if used with other corticosteroids (oral or inhaled).
CYP3A4 inhibitors (e.g., Ritonavir, Ketoconazole)
Concurrent use with strong CYP3A4 inhibitors can significantly increase systemic fluticasone exposure, leading to increased risk of systemic corticosteroid side effects (e.g., Cushing's syndrome, adrenal suppression).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Acute overdose is unlikely to cause severe problems due to low systemic absorption. However, chronic overdose may lead to systemic corticosteroid effects like adrenal suppression. Treatment is supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is not known whether fluticasone propionate is excreted in human milk; use with caution in nursing mothers.
