Description
Overview of the medicine
Asmalin is a fast-acting bronchodilator used to relieve symptoms of asthma and chronic obstructive pulmonary disease (COPD). It works by relaxing the muscles in the airways, allowing for easier breathing.
Uses & Indications
Dosage
Adults
Oral: 2-4 mg, 3-4 times daily. Inhalation (MDI): 1-2 puffs (100-200 mcg) as needed, usually up to 4 times a day. Nebuliser solution: 2.5-5 mg via nebuliser.
Elderly
Similar to adults, but initial dose may be lower. Use with caution due to potential increased sensitivity to beta-adrenergic stimulation.
Renal_impairment
No specific dose adjustment is usually required for oral forms. For severe impairment, consider lower doses and monitor for side effects.
How to Take
Oral tablets can be taken with or without food. Inhalers require proper technique for effective delivery; patients should be instructed on how to use them correctly. Nebuliser solution is administered via a nebuliser machine.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors in the smooth muscle of the bronchial tree. This stimulation leads to an increase in intracellular cyclic AMP, which in turn causes relaxation of bronchial smooth muscle and inhibition of mediator release from mast cells.

Excretion
Primarily excreted in the urine as unchanged drug and its sulfate conjugate.
Half life
Oral: 4-6 hours; Inhalation: 3-5 hours (systemic)
Absorption
Well absorbed from the gastrointestinal tract (oral) and lungs (inhalation). Oral bioavailability is about 50%.
Metabolism
Undergoes extensive hepatic metabolism, primarily to an inactive sulfate conjugate.
Side Effects
Contraindications
- •Hypersensitivity to salbutamol or any component of the formulation.
- •Patients with pre-existing cardiac conditions should use with caution.
Drug Interactions
Diuretics
Concomitant use with potassium-depleting diuretics (e.g., loop or thiazide diuretics) may increase the risk of hypokalemia.
Beta-blockers
May antagonize the effects of salbutamol and can produce severe bronchospasm in asthmatic patients. Should not be used concurrently.
MAOIs and TCAs
Use with extreme caution in patients being treated with monoamine oxidase inhibitors or tricyclic antidepressants as the action of salbutamol on the vascular system may be potentiated.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, headache, palpitations, and hypokalemia. Management involves supportive therapy, monitoring of cardiac function and serum potassium levels.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Salbutamol is likely to be excreted in breast milk; caution is advised when administering to a nursing mother.
