Description
Overview of the medicine
Atorvastatin is a lipid-lowering medication used to reduce levels of 'bad' cholesterol (low-density lipoprotein, or LDL) and triglycerides in the blood, while increasing levels of 'good' cholesterol (high-density lipoprotein, or HDL). It is used to prevent cardiovascular diseases such as heart attacks and strokes.
Uses & Indications
Dosage
Adults
The starting dose is usually 10 mg once daily. The dose can be adjusted based on LDL-C levels and patient response, up to a maximum of 80 mg once daily. Doses should be individualized.
Elderly
No dosage adjustment is required in elderly patients (≥65 years of age).
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Atova 10 mg Tablet should be taken orally, once daily, with or without food, at any time of day. However, it is advisable to take it at the same time each day.
Mechanism of Action
Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the early rate-limiting step in cholesterol biosynthesis. This inhibition results in a decrease in hepatic cholesterol synthesis and a compensatory increase in the number of hepatic LDL receptors on the cell surface, which enhances the uptake and catabolism of LDL. It also reduces VLDL and triglyceride production.
Pharmacokinetics
Onset
Significant lipid-lowering effects typically within 2 weeks, with maximum effect by 4 weeks.
Excretion
Excreted primarily in bile following hepatic and/or extrahepatic metabolism; less than 2% of a dose is recovered in urine.
Half life
The elimination half-life of atorvastatin is approximately 14 hours; however, the half-life of inhibitory activity for HMG-CoA reductase is 20 to 30 hours due to active metabolites.
Absorption
Rapidly absorbed after oral administration; absolute bioavailability is approximately 14%. Peak plasma concentrations (Cmax) occur within 1 to 2 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 3A4 (CYP3A4) to active ortho- and parahydroxylated metabolites and various beta-oxidation products.
Side Effects
Contraindications
- •Active liver disease or unexplained persistent elevations of serum transaminases
- •Pregnancy and lactation
- •Hypersensitivity to atorvastatin or any component of the formulation
- •Concomitant use with glecaprevir/pibrentasvir
Drug Interactions
Warfarin
May increase international normalized ratio (INR). Monitor INR closely.
Gemfibrozil
Increased risk of myopathy/rhabdomyolysis. Concomitant use should be avoided.
Oral Contraceptives
May increase AUC of norethindrone and ethinyl estradiol.
CYP3A4 Inhibitors (e.g., Cyclosporine, Clarithromycin, Itraconazole)
Increased risk of myopathy/rhabdomyolysis. Dose adjustment or alternative therapy may be needed.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
There is no specific treatment for atorvastatin overdose. In the event of an overdose, the patient should be treated symptomatically, and supportive measures instituted as required. Due to extensive drug binding to plasma proteins, hemodialysis is not expected to significantly enhance atorvastatin clearance.
Pregnancy & Lactation
Atorvastatin is contraindicated during pregnancy (Pregnancy Category X) due to the potential for fetal harm. Women of childbearing potential should use adequate contraception. It is also contraindicated during lactation as it may be excreted in breast milk.
