Overview of the medicine
AV 5 mg Tablet contains Amlodipine, a calcium channel blocker, used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, making it easier for the heart to pump blood.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, maximum 10 mg once daily. Dosage should be adjusted according to patient response.
Elderly
Initial dose 2.5 mg once daily, gradually increase if needed, monitor carefully.
Renal_impairment
No dosage adjustment required.
How to Take
Oral administration. Can be taken with or without food.
Mechanism of Action
Amlodipine is a dihydropyridine calcium antagonist (calcium ion antagonist or slow channel blocker) that inhibits the transmembrane influx of calcium ions into cardiac and vascular smooth muscle. This leads to vasodilation and reduction in peripheral vascular resistance, lowering blood pressure. In angina, it increases myocardial oxygen delivery.

Excreted predominantly by the kidneys (approx. 60% as inactive metabolites, 10% as unchanged drug).

Extensively metabolized in the liver to inactive metabolites.

Contraindications
- Hypersensitivity to amlodipine or other dihydropyridines
- Severe hypotension
- Cardiogenic shock
- Unstable angina (unless treated with beta-blockers)
Drug Interactions
Sildenafil
Increased risk of hypotension, use with caution.
Simvastatin
Increased risk of myopathy/rhabdomyolysis; limit simvastatin dose to 20 mg/day.
CYP3A4 Inducers (e.g., Rifampicin, Carbamazepine)
May decrease amlodipine exposure, monitor for reduced efficacy.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin)
May increase amlodipine exposure, monitor for hypotension.

Use only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, use with caution in nursing mothers.
