Description
Overview of the medicine
Avanafil is a phosphodiesterase-5 (PDE5) inhibitor used to treat erectile dysfunction (ED). It helps men achieve and maintain an erection by increasing blood flow to the penis when sexually stimulated.
Uses & Indications
Dosage
Adults
The recommended starting dose of Avidro is 15 mg, taken orally as needed, approximately 15 to 30 minutes before sexual activity. The dose may be increased or decreased based on efficacy and tolerability, but 15 mg is the prescribed strength here. Do not exceed one dose per day.
Elderly
No specific dosage adjustment is required for elderly patients based on age alone, but caution is advised, and lower starting doses may be considered if co-morbidities or concomitant medications warrant.
Renal_impairment
No dose adjustment is required for patients with mild to moderate renal impairment. Use with caution in severe renal impairment, and specific dosage recommendations might vary.
How to Take
Oral, with or without food, approximately 15-30 minutes before sexual activity.
Mechanism of Action
Avanafil selectively inhibits cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5) in the corpus cavernosum of the penis. When sexual stimulation causes local release of nitric oxide, PDE5 inhibition by avanafil increases cGMP levels, leading to smooth muscle relaxation and inflow of blood into the penis, resulting in an erection.
Pharmacokinetics
Onset
~15-30 minutes.
Excretion
Excreted predominantly via feces (62%) and to a lesser extent via urine (36%) as metabolites.
Half life
~6-17 hours (terminal half-life).
Absorption
Rapidly absorbed, Cmax reached in 30-45 minutes. Bioavailability ~20-40%.
Metabolism
Primarily hepatic, mainly by CYP3A4 and to a lesser extent by CYP2C9.
Side Effects
Contraindications
- Concomitant use with nitrates (e.g., nitroglycerin)
- Hypersensitivity to avanafil or any component
- Severe cardiovascular disease (e.g., recent MI or stroke, unstable angina)
- Severe hepatic impairment
- End-stage renal disease requiring dialysis
- Concomitant use with alpha-blockers in patients with uncontrolled hypertension
- Concomitant use with strong CYP3A4 inhibitors (e.g., ketoconazole, ritonavir)
Drug Interactions
Nitrates
Potentiation of hypotensive effects, potentially life-threatening. Absolutely contraindicated.
Alpha-blockers
Increased risk of symptomatic hypotension. Use with caution, initiate at lowest dose.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Ritonavir)
Significantly increases avanafil exposure. Concomitant use is contraindicated.
Moderate CYP3A4 Inhibitors (e.g., Erythromycin, Diltiazem)
Increased avanafil exposure, dosage adjustment may be needed.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as avanafil is highly bound to plasma proteins and not significantly renally eliminated.
Pregnancy & Lactation
Not indicated for use in women.
