Description
Overview of the medicine
Avolose (Lactulose) is a synthetic disaccharide used to treat constipation and hepatic encephalopathy. It works by drawing water into the colon, softening stools, and reducing ammonia levels in the blood.
Uses & Indications
Dosage
Adults
For constipation: Initially 15-45 ml daily, adjusted according to response. For hepatic encephalopathy: Initially 30-45 ml, 3-4 times daily, adjusted to produce 2-3 soft stools per day.
Elderly
Same as adults, but may start with lower doses and adjust cautiously.
Renal_impairment
No specific dose adjustment required as it is minimally absorbed.
How to Take
Can be taken orally, either undiluted or diluted with water, fruit juice, or milk. For constipation, usually taken once daily, often in the morning. For hepatic encephalopathy, doses are spread throughout the day.
Mechanism of Action
Lactulose is poorly absorbed from the gastrointestinal tract. In the colon, it is metabolized by bacterial flora into low molecular weight organic acids, which increases osmotic pressure, drawing water into the colon and softening the stool. In hepatic encephalopathy, these acids also lower the pH of the colon, which traps ammonia (NH3) as ammonium (NH4+) ions, preventing its absorption into the bloodstream and facilitating its excretion.

Onset
Laxative effect: 24-48 hours. Hepatic encephalopathy: 24-72 hours.
Excretion
Primarily excreted unchanged in feces; absorbed portion excreted in urine.
Half life
Not significant due to minimal absorption.
Absorption
Poorly absorbed (less than 3%) from the GI tract.
Metabolism
Metabolized by colonic bacteria into lactic acid, acetic acid, etc.
Side Effects
Contraindications
- •Hypersensitivity to lactulose or any component of the formulation

- •Intestinal obstruction (e.g., paralytic ileus, colon obstruction)
- •Acute abdominal pain of unknown cause
Drug Interactions
Antacids
May interfere with the desired pH reduction in the colon.
Broad-spectrum antibiotics
May reduce the efficacy of lactulose by altering colonic bacterial flora.
Storage
Store below 30°C, away from light and moisture. Do not refrigerate or freeze.
Overdose
Symptoms of overdose include abdominal pain, diarrhea, electrolyte imbalance (especially hypokalemia). Treatment involves discontinuing the drug, correcting fluid and electrolyte imbalances.

Pregnancy Category B. Generally considered safe during pregnancy and lactation as it is minimally absorbed. Consult a doctor.
