Description
Overview of the medicine
Axeclav 500 mg Tablet is a combination antibiotic containing Amoxicillin and Clavulanic Acid. It is used to treat a wide range of bacterial infections by killing the bacteria and preventing them from becoming resistant to the antibiotic.
Uses & Indications
Dosage
Adults
One Axeclav 500 mg/125 mg tablet (625 mg total) every 8 hours, or one Axeclav 875 mg/125 mg tablet every 12 hours, for 7-10 days, depending on the severity of the infection.
Elderly
No dosage adjustment needed unless there is significant renal impairment. Monitor renal function.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For creatinine clearance between 10-30 ml/min, one 500 mg/125 mg tablet every 12 hours. For <10 ml/min, one 500 mg/125 mg tablet every 24 hours.
How to Take
Take the tablet with meals or a light snack to reduce potential gastrointestinal discomfort and enhance absorption. Swallow whole with water, do not crush or chew.
Mechanism of Action
Amoxicillin works by inhibiting bacterial cell wall synthesis, leading to bacterial lysis and death. Clavulanic acid is a beta-lactamase inhibitor that protects amoxicillin from degradation by beta-lactamase enzymes produced by resistant bacteria, thereby extending amoxicillin's antibacterial spectrum.
Pharmacokinetics
Onset
Antibacterial effects typically begin within 1-2 hours of administration.
Excretion
Both components are primarily excreted unchanged by the kidneys, with some renal tubular secretion. A small amount is excreted in feces.
Half life
The elimination half-life is approximately 1-1.5 hours for amoxicillin and about 1 hour for clavulanic acid in healthy adults.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration, with peak plasma concentrations occurring after approximately 1-2 hours.
Metabolism
Amoxicillin is partially metabolized to an inactive penicilloic acid. Clavulanic acid is extensively metabolized to various inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to penicillin or any component of Axeclav.
- History of severe hypersensitivity reaction (e.g., anaphylaxis) to any other beta-lactam antibiotic.
- History of jaundice or hepatic dysfunction associated with previous co-amoxiclav therapy.
Drug Interactions
Warfarin
May prolong prothrombin time; close monitoring of INR is recommended.
Probenecid
Increases and prolongs amoxicillin blood levels by reducing renal tubular secretion.
Allopurinol
Increased incidence of skin rash, especially in hyperuricemic patients.
Methotrexate
Increased methotrexate toxicity due to reduced renal excretion.
Oral Contraceptives
May reduce the efficacy of oral contraceptives; advise additional non-hormonal contraception.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal symptoms (nausea, vomiting, diarrhea) and disturbances of fluid and electrolyte balance. In isolated cases, renal failure, convulsions, and crystalluria have been observed. Treatment is symptomatic and supportive, including maintaining fluid and electrolyte balance. Hemodialysis may be used to remove amoxicillin and clavulanic acid from circulation.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Both amoxicillin and clavulanic acid are excreted into breast milk; use with caution in nursing mothers, as it may cause sensitization in the infant.
