Axofyl 400 mg Tablet contains Cefixime, a third-generation cephalosporin antibiotic used to treat various bacterial infections including those of the respiratory tract, urinary tract, ear, nose, and throat.
Uses & Indications
Dosage
Adults
400 mg once daily or 200 mg every 12 hours for 7-14 days, depending on the infection.
Elderly
No dosage adjustment necessary based solely on age, unless renal function is significantly impaired.
Renal_impairment
Creatinine clearance 21-60 mL/min: 300 mg daily. Creatinine clearance less than 20 mL/min: 200 mg daily. Hemodialysis or CAPD: 200 mg daily.
How to Take
Axofyl 400 mg Tablet should be taken orally, with or without food. It is recommended to complete the full course of treatment as prescribed by the doctor, even if symptoms improve.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which prevents the cross-linking of peptidoglycan chains, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
Peak plasma concentrations reached within 2-6 hours.
Excretion
Excreted by both renal and non-renal mechanisms, approximately 50% via urine within 24 hours.
Half life
About 3-4 hours in healthy adults.
Absorption
Approximately 40-50% orally absorbed. Food slightly prolongs Tmax but does not significantly alter Cmax or AUC.
Metabolism
Not extensively metabolized; primarily eliminated as unchanged drug.
Side Effects
Contraindications
- •Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- •History of hypersensitivity to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase Cefixime peak plasma levels and AUC.
Probenecid
Increases Cefixime plasma concentration by decreasing renal excretion.
Carbamazepine
Elevated carbamazepine levels reported when co-administered.
Oral Contraceptives
May reduce efficacy of oral contraceptives (use alternative contraception).
Warfarin and Anticoagulants
May increase prothrombin time with or without bleeding, monitor INR.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of severe overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, Cefixime should be used during pregnancy only if clearly needed. Excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
