Overview of the medicine
Azaltic 200 mg Suspension is a macrolide antibiotic used to treat a wide variety of bacterial infections, including respiratory tract infections, skin infections, and sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days. For Chlamydia, a single 1 gram dose.
Elderly
No specific dose adjustment is needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment (CrCl >10 ml/min). Use with caution in severe renal impairment (CrCl <10 ml/min).
How to Take
Shake the suspension well before each use. Can be taken with or without food. However, taking it with food may help reduce gastrointestinal upset. Complete the full course of treatment even if symptoms improve.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms and, thus, inhibits RNA-dependent protein synthesis. This is generally considered to be bacteriostatic, but high concentrations can be bactericidal.

Onset
Antibacterial effects usually within 2-3 hours of first dose.
Excretion
Primarily excreted unchanged in bile; a small amount is excreted via urine.
Half life
Terminal half-life is 2-4 days (approximately 68 hours).
Absorption
Rapidly absorbed from the gastrointestinal tract, but bioavailability is reduced by food. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Primarily metabolized in the liver by N-demethylation.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Digoxin
Increased serum digoxin levels.
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased azithromycin serum concentrations.
Cyclosporine
Increased cyclosporine concentrations, monitor levels.
Antacids (containing aluminum or magnesium)
Reduced peak serum levels of azithromycin. Take azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store at room temperature (15-30°C) or refrigerated (2-8°C) and discard unused portion after 5 days.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and supportive care are recommended.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Excreted into breast milk; use with caution during lactation, benefits should outweigh risks.
