Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For most infections: 500 mg orally once daily for 3 days, or 500 mg as a single dose on day 1 followed by 250 mg once daily for 4 days. For Chlamydia: 1 gram as a single dose.
Elderly
No specific dosage adjustment is generally required for elderly patients, but caution is advised in those with severe renal or hepatic impairment.
Renal_impairment
No dosage adjustment is needed for mild to moderate renal impairment (creatinine clearance >10 mL/min). Use with caution in severe renal impairment (creatinine clearance <10 mL/min).
How to Take
Azithromycin tablets can be taken orally with or without food. It should be taken as a single daily dose.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby preventing peptide translocation.
Pharmacokinetics
Onset
Peak plasma concentrations are reached within 2-3 hours.
Excretion
Primarily excreted unchanged in the bile; a minor portion is excreted renally.
Half life
The terminal elimination half-life is 2-4 days, attributed to extensive tissue uptake and slow release.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Partially metabolized in the liver by N-demethylation.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased serum digoxin levels have been reported with concomitant use of macrolide antibiotics; monitor digoxin levels.
Warfarin
Azithromycin may potentiate the anticoagulant effects of warfarin, requiring careful monitoring of prothrombin time.
Antacids containing aluminum or magnesium
May decrease the peak plasma concentrations of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves gastric lavage and general symptomatic and supportive measures as clinically indicated.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. Excreted in breast milk; use with caution in nursing mothers and only if the potential benefit outweighs the potential risk to the infant. Consult your doctor.
