Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases. This 200 mg/5 ml suspension is primarily formulated for pediatric use.
Uses & Indications
Dosage
Adults
Rarely used for adults in suspension form; adult dosage for azithromycin (as tablets/capsules) is typically 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days.
Elderly
Similar to adult dosage for azithromycin; no dose adjustment is generally required for elderly patients with normal renal and hepatic function. For suspension, follow pediatric guidelines if applicable for specific indications.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment (creatinine clearance >10 mL/min). Caution is advised in severe renal impairment.

Azi-S 200 mg Suspension is for oral administration. It can be taken with or without food. Ensure the bottle is shaken well before each use. Use the provided measuring device to ensure accurate dosing.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translocation of peptides.

Onset of action typically occurs within 1-2 hours after administration.
Excretion
The primary route of excretion is biliary, with a small portion (approximately 6-11%) excreted unchanged in the urine over 72 hours.
Half life
The terminal elimination half-life is prolonged, ranging from 2 to 4 days, allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract, but bioavailability is approximately 37% due to extensive tissue distribution. Absorption is generally unaffected by food for the suspension.
Metabolism
Primarily metabolized in the liver, but limited metabolism occurs. It undergoes demethylation in the liver.
Side Effects
Contraindications
- •Known hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum digoxin levels. Monitor digoxin levels if co-administered.
Warfarin
May enhance the anticoagulant effects of warfarin. Close monitoring of prothrombin time is recommended.
Nelfinavir
Increased azithromycin serum concentrations. No dosage adjustment is recommended.
Antacids (containing aluminum or magnesium)
Decreased peak plasma concentration of azithromycin when co-administered. Administer azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store the unreconstituted powder below 30°C, protected from light and moisture. Keep the reconstituted suspension below 30°C, and discard any unused portion after 5 days. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose are typically an extension of common side effects, such as severe nausea, vomiting, and diarrhea. Management includes symptomatic and supportive measures as clinically indicated. Gastric lavage and general supportive care may be instituted.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm to the fetus. There are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Azithromycin is excreted in breast milk; caution should be exercised when administered to a nursing mother.
