Description
Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat a wide variety of bacterial infections. It works by stopping the growth of bacteria. This medicine is also used to prevent certain bacterial infections.
Uses & Indications
Dosage
Adults
For most infections: 500 mg orally once daily for 3 days, OR 500 mg orally as a single dose on day 1, followed by 250 mg once daily for 4 days. For uncomplicated chlamydial infections: 1 gram orally as a single dose.
Elderly
No dosage adjustment is generally required for elderly patients, unless severe renal or hepatic impairment is present.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment (CrCl < 10 mL/min).
How to Take
Azi-S 500 mg Tablet can be taken orally with or without food. To minimize gastrointestinal upset, it may be taken with food.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting microbial protein synthesis. It does not affect nucleic acid synthesis.
Pharmacokinetics
Onset
Clinical effects may be observed within hours to days, depending on the infection.
Excretion
Primarily excreted unchanged in the bile; a small amount is excreted via urine (approximately 6% of the dose).
Half life
Approximately 68 hours (terminal half-life), allowing for once-daily dosing.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations are achieved within 2-3 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites by N-demethylation.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- •History of cholestatic jaundice or hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased serum digoxin levels.
Warfarin
Increased anticoagulant effect, monitor INR.
Nelfinavir
Increased azithromycin concentration.
Antacids (containing aluminum or magnesium)
Decreased peak plasma levels of azithromycin; administer azithromycin at least 2 hours before or after antacids.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, abdominal discomfort, and reversible hearing loss. Treatment is symptomatic and supportive; no specific antidote is available. Gastric lavage and general supportive measures are indicated.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. Excreted in breast milk; caution should be exercised when azithromycin is administered to a nursing woman.
