Overview of the medicine
Azithromycin is a macrolide antibiotic used to treat various bacterial infections, including those of the respiratory tract, skin, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days, or 500 mg as a single dose on Day 1, followed by 250 mg once daily on Days 2-5. For Chlamydia trachomatis: 1 gram as a single oral dose.
Elderly
No specific dosage adjustment is required for elderly patients, but caution is advised in those with cardiac conditions due to potential QT prolongation.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment (creatinine clearance >40 mL/min). Use with caution in severe renal impairment.
How to Take
Azilab tablets can be taken orally, with or without food. Taking it with food may help reduce gastrointestinal upset. Swallow the tablet whole with a glass of water.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translocation of peptides.
Pharmacokinetics
Onset
Within 2-3 hours of administration for clinical effect.
Excretion
Primarily via bile (feces) as unchanged drug and metabolites; minor renal excretion (approximately 6% of dose).
Half life
The terminal elimination half-life is 2 to 4 days.
Absorption
Rapidly absorbed from the GI tract. Bioavailability is approximately 37%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Partially metabolized in the liver to inactive metabolites. No significant first-pass metabolism.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Warfarin
Increased anticoagulant effect. Monitor prothrombin time/INR.
Nelfinavir
Increased serum concentrations of azithromycin. Monitor for known azithromycin side effects.
Antacids (containing aluminum or magnesium)
Decreased peak plasma levels of azithromycin. Administer azithromycin at least 1 hour before or 2 hours after antacids.
QT-prolonging drugs (e.g., Class IA and III antiarrhythmics, cisapride, terfenadine)
Increased risk of QT interval prolongation and Torsades de Pointes. Avoid coadministration.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of azithromycin overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and general supportive measures may be indicated as needed.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. However, there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Azithromycin is excreted in breast milk; use with caution in breastfeeding mothers.
