Mainly excreted unchanged in bile; a small amount is excreted in urine.
Half life
Terminal elimination half-life is 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.
Metabolism
Primarily metabolized in the liver, but a significant portion is excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use
Drug Interactions
Warfarin
Increased anticoagulant effect; monitor PT/INR closely.
Nelfinavir
Increased azithromycin concentrations.
Antacids (containing aluminum or magnesium)

QT prolonging drugs (e.g., amiodarone, quinidine)
Increased risk of cardiac arrhythmias. Avoid co-administration if possible.
Storage
Store dry powder below 30°C, protected from light and moisture. After reconstitution, store at room temperature (15-30°C) and discard unused portion after the specified period (e.g., 5-10 days).
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and activated charcoal may be considered soon after ingestion.
