Albendazole is a broad-spectrum anthelmintic used to treat various parasitic worm infections. It is a benzimidazole derivative.
Uses & Indications
Dosage
Adults
For common intestinal worms (Ascariasis, Hookworm, Enterobiasis, Trichuriasis): 400 mg as a single dose. For Strongyloidiasis/Taeniasis: 400 mg once daily for 3 days. For Hydatid disease/Neurocysticercosis: Dosage varies, often 400 mg twice daily with food for specific durations (e.g., 8-30 days for neurocysticercosis, cycles for hydatid).
Elderly
No specific dose adjustment is generally required, but close monitoring of renal and hepatic function is advised, especially during prolonged treatment.
Renal_impairment
No dose adjustment is typically needed for patients with renal impairment, but caution is advised in severe cases due to potential accumulation of metabolites.
How to Take
The tablet should be chewed or crushed and swallowed with water. For optimal absorption, it is recommended to take it with a fatty meal.
Mechanism of Action
Albendazole selectively binds to tubulin, inhibiting microtubule polymerization, which disrupts parasite cell structure and impairs glucose uptake, leading to depletion of glycogen stores and eventually parasite death.
Pharmacokinetics
Onset
Systemic effects are generally observed within hours. Therapeutic effects against various worm infections depend on the worm type and location.
Excretion
Primarily excreted in the urine as metabolites (less than 1% as unchanged drug). Some biliary excretion also occurs.
Half life
The plasma half-life of the active metabolite, albendazole sulfoxide, is approximately 8-12 hours.
Absorption
Poorly absorbed (<5%) from the gastrointestinal tract. Absorption is significantly increased (up to 5-fold) when taken with a fatty meal.
Metabolism
Undergoes rapid and extensive first-pass metabolism in the liver to the primary active metabolite, albendazole sulfoxide, which is then further metabolized to inactive sulfone and other metabolites.
Side Effects
Contraindications
- •Known hypersensitivity to Albendazole or other benzimidazole derivatives.
- •Pregnancy (especially during the first trimester due to potential teratogenicity; adequate contraception should be used for women of childbearing potential during and for at least one month after treatment).
Drug Interactions
Cimetidine, Dexamethasone, Praziquantel
May increase the plasma concentrations of albendazole sulfoxide, potentially leading to increased side effects.
Ritonavir, Phenytoin, Carbamazepine, Phenobarbital
May decrease the plasma concentrations of albendazole sulfoxide, potentially reducing its efficacy.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
In cases of overdose, gastric lavage may be considered depending on the ingested amount and time. General supportive measures and symptomatic treatment should be administered. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C/D (depending on exposure). Albendazole should not be used in pregnant women, especially during the first trimester, unless the potential benefit justifies the potential risk to the fetus. Women of childbearing potential should use effective contraception during and for at least one month after treatment. It is unknown if albendazole or its metabolites are excreted in human breast milk. Caution should be exercised when administered to a nursing woman.
