Description
Overview of the medicine
Baclomark (Baclofen) is a centrally acting skeletal muscle relaxant used to treat spasticity resulting from multiple sclerosis, spinal cord injuries, or other spinal cord diseases.
Uses & Indications
Dosage
Adults
Initial dose: 5 mg three times daily (TID). Gradually increase by 5 mg TID every 3 days until optimal effect is achieved. Max dose: 80 mg/day.
Elderly
Start with a lower dose (e.g., 5 mg BID) and titrate slowly due to increased sensitivity and potential for adverse effects.
Renal_impairment
Dose reduction is necessary. Start with 5 mg daily and adjust based on clinical response and creatinine clearance.
How to Take
Take orally with food or milk to minimize gastrointestinal discomfort. Do not discontinue abruptly.
Mechanism of Action
Baclofen acts as a GABA-B receptor agonist, leading to hyperpolarization of neurons and reduced excitatory neurotransmitter release, which in turn reduces muscle spasticity.

Onset
Onset of action is usually within 1 hour.
Excretion
Primarily excreted unchanged by the kidneys (70-80%) within 72 hours.
Half life
The elimination half-life is 3-4 hours.
Absorption
Rapidly and extensively absorbed from the gastrointestinal tract. Peak plasma concentrations are achieved within 2-3 hours.
Metabolism
Approximately 5% of the dose is metabolized in the liver. The main metabolite is β-(p-chlorophenyl)-γ-hydroxybutyric acid.
Side Effects
Contraindications

Drug Interactions
MAO inhibitors
May increase the CNS depressant effects of baclofen (use with caution).
Antihypertensives

Storage

Overdose
Symptoms include drowsiness, severe muscle weakness, vomiting, respiratory depression, coma, and seizures. Management involves supportive care, gastric lavage if recent ingestion, and symptomatic treatment. Avoid abrupt withdrawal.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit outweighs the potential risk to the fetus. Baclofen is excreted in breast milk; caution should be exercised when administered to a nursing mother.
