Description
Overview of the medicine
Cefixime is a broad-spectrum cephalosporin antibiotic used to treat various bacterial infections, especially in children. It works by inhibiting bacterial cell wall synthesis.
Uses & Indications
Dosage
Adults
Not typically used for adults in this strength; adult dosage is usually 200-400 mg daily.
Elderly
No dose adjustment required unless severe renal impairment.
Children
6 months to 12 years: 8 mg/kg/day once daily or 4 mg/kg every 12 hours. The usual dose for 125mg/5ml suspension is based on weight, typically 5ml once daily for children weighing 12.5-25kg. Do not exceed adult dose.
Renal_impairment
Dose adjustment necessary for creatinine clearance < 60 mL/min. For CrCl 21-60 mL/min, 75% of standard dose. For CrCl < 20 mL/min, 50% of standard dose.
How to Take
Oral administration. May be taken with or without food. Shake the reconstituted suspension well before each use.
Mechanism of Action
Cefixime inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved 2-6 hours after administration.
Excretion
Approximately 50% of an absorbed dose is excreted unchanged in the urine over 24 hours. Some excretion also occurs via bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% orally absorbed. Food has minimal effect on absorption.
Metabolism
Not extensively metabolized in the liver.
Side Effects
Contraindications
- •Known hypersensitivity to cefixime or any other cephalosporin antibiotics.
- •Known hypersensitivity to penicillin antibiotics (due to potential cross-reactivity).
Drug Interactions
Warfarin
May increase prothrombin time (PT) and International Normalized Ratio (INR), increasing bleeding risk. Monitor coagulation parameters.
Probenecid
Increases cefixime plasma concentrations by decreasing renal excretion.
Carbamazepine
May increase plasma concentrations of carbamazepine. Monitor carbamazepine levels.
Storage
Store dry powder below 30°C, protect from light and moisture. Keep out of reach of children. After reconstitution, store at room temperature (not exceeding 30°C) or in a refrigerator (2-8°C) and use within 7-14 days, do not freeze.
Overdose
Gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Supportive therapy should be administered.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Cefixime is excreted in small amounts in breast milk; use with caution in nursing mothers.
