Description
Overview of the medicine
BC-Card is a beta-blocker primarily used to treat high blood pressure, angina, and chronic stable heart failure. It works by relaxing blood vessels and slowing the heart rate, improving blood flow and reducing blood pressure.
Uses & Indications
Dosage
Adults
Initial dose 2.5-5 mg once daily, adjusted based on response up to a maximum of 20 mg daily.
Elderly
Initial dose may be lower, typically 2.5 mg once daily, with careful titration.
Renal_impairment
For severe renal impairment (CrCl < 20 mL/min), initial dose 2.5 mg daily. Max 10 mg daily.
How to Take
Take orally, once daily in the morning with or without food. Swallow the tablet whole with water.
Mechanism of Action
Bisoprolol is a highly selective beta1-adrenergic receptor blocker. It decreases heart rate and cardiac output, thereby lowering blood pressure. It also reduces myocardial oxygen demand.
Pharmacokinetics
Onset
1-2 hours
Excretion
Excreted almost equally via renal and hepatic pathways. About 50% unchanged in urine.
Half life
9-12 hours
Absorption
Well absorbed from the GI tract (over 90%). Bioavailability is approximately 90%.
Metabolism
Approximately 50% metabolized in the liver to inactive metabolites.

NSAIDs
May reduce antihypertensive effects.
Clonidine
Risk of rebound hypertension if clonidine is stopped abruptly while on bisoprolol.
Insulin and oral antidiabetics
May mask symptoms of hypoglycemia.
Antiarrhythmic drugs (e.g., Disopyramide, Amiodarone)
Potentiation of cardiodepressant effects.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and hypotension.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
