Overview of the medicine
Benmet 200 mg Suspension contains Metronidazole, an antibiotic and antiprotozoal medication used to treat a wide variety of infections caused by susceptible bacteria and parasites, including amoebiasis, giardiasis, trichomoniasis, and anaerobic bacterial infections.
Uses & Indications
Dosage
Adults
Amoebiasis: 400-800 mg 3 times daily for 5-10 days. Giardiasis: 400 mg 3 times daily for 5 days. Trichomoniasis: 200 mg 3 times daily for 7 days or 2 g as a single dose.
Elderly
No specific dose reduction required, but caution with hepatic impairment. Dosage as per adult, adjust based on renal/hepatic function.
Renal_impairment
No dosage adjustment needed for mild to moderate impairment. For severe impairment (CrCl <10 mL/min), dose reduction may be considered, e.g., 50% of the usual dose.
How to Take
Take orally, preferably with or after food, to minimize gastrointestinal upset. Shake the bottle well before each use.
Mechanism of Action
Metronidazole acts by inhibiting nucleic acid synthesis. Its nitro group is reduced in anaerobic bacteria and sensitive protozoa to reactive intermediates that disrupt DNA structure and inhibit bacterial and protozoal growth.
Pharmacokinetics
Onset
Within 2-3 days for most infections, but depends on infection type and severity.
Excretion
Mainly through the urine (60-80%), with a smaller amount excreted in feces.
Half life
Approximately 8 hours (range 6 to 12 hours).
Absorption
Well absorbed orally, with peak plasma concentrations occurring within 1-2 hours after administration.
Metabolism
Primarily hepatic by oxidation, producing active hydroxyl metabolites.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives
- •First trimester of pregnancy (relative contraindication for trichomoniasis)
Drug Interactions
Alcohol
Disulfiram-like reaction (flushing, palpitations, nausea, vomiting, headache, hypotension). Avoid alcohol during and for at least 3 days after treatment.
Lithium
Increases plasma lithium levels, leading to toxicity. Monitor lithium levels.
Warfarin
Potentiates the anticoagulant effect, increasing bleeding risk. Monitor INR closely.
Cimetidine
Increases metronidazole levels, potentially enhancing side effects.
Phenytoin/Phenobarbital
Decreases metronidazole levels, reducing efficacy.
Storage
Store below 30°C in a dry place, away from light. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, and mild disorientation. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Generally avoided during the first trimester. Use with caution in later trimesters only if clearly needed. Metronidazole is excreted into breast milk; breastfeeding should be temporarily discontinued during treatment and for 12-24 hours after the last dose.
