Description
Overview of the medicine
Betapro-25 mg Tablet contains Metoprolol Tartrate, a beta-blocker primarily used to treat high blood pressure, angina, and certain heart rhythm disorders. It works by relaxing blood vessels and slowing heart rate, improving blood flow and reducing blood pressure.
Uses & Indications
Dosage
Adults
Hypertension: Initially 50-100 mg daily in 1 or 2 divided doses. Angina: 50-100 mg twice daily. The 25mg dose is often used for initial titration or specific conditions.
Elderly
Similar to adult dosage, but caution with initial lower doses due to potential for increased sensitivity and reduced elimination.
Renal_impairment
No dosage adjustment usually required for mild to moderate renal impairment. Use with caution in severe impairment.
How to Take
Take orally, usually with or immediately after a meal to increase absorption and reduce gastric irritation. Do not crush or chew the tablet.
Mechanism of Action
Metoprolol is a cardioselective beta-1 adrenergic blocking agent. It selectively blocks beta-1 receptors in the heart, reducing heart rate, myocardial contractility, and cardiac output. At higher doses, it may also block beta-2 receptors.
Pharmacokinetics
Onset
Within 1 hour (oral)
Excretion
Mainly excreted in urine as metabolites, with a small portion as unchanged drug.
Half life
3-7 hours
Absorption
Rapid and complete absorption from the GI tract (approx. 95%), but undergoes significant first-pass metabolism, leading to a bioavailability of about 50%.
Metabolism
Extensively metabolized by the liver, primarily via CYP2D6.
Side Effects
Contraindications
- Sinus bradycardia
- Second or third-degree heart block
- Cardiogenic shock
- Decompensated heart failure
- Sick sinus syndrome (unless a pacemaker is in place)
- Severe peripheral arterial disease
Drug Interactions
Digoxin
Increased risk of bradycardia.
Other antihypertensives
Additive hypotensive effects.
NSAIDs (e.g., Ibuprofen)
May reduce the hypotensive effect of beta-blockers.
Calcium channel blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and AV block.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe bradycardia, hypotension, cardiac insufficiency, bronchospasm, and cardiac arrest. Management involves gastric lavage, activated charcoal, and symptomatic supportive care (e.g., atropine for bradycardia, vasopressors for hypotension, glucagon, or pacemaking).
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Metoprolol is excreted in breast milk; caution advised during lactation.
