Description
Overview of the medicine
Betason-N eye ointment is a combination medication containing a corticosteroid (Betamethasone) and an antibiotic (Neomycin). It is used to treat inflammatory eye conditions where there is a risk of bacterial infection or where a bacterial infection is already present.
Uses & Indications
Dosage
Adults
Apply a small amount (about 1 cm strip) into the conjunctival sac 2-3 times daily, or as directed by a physician. Treatment duration should be limited to avoid steroid-related side effects.
Elderly
No specific dosage adjustment required, but monitor for increased susceptibility to side effects.
Renal_impairment
Not applicable for topical ophthalmic use as systemic absorption is minimal. However, caution with severe renal impairment if significant systemic absorption is suspected with prolonged use.
How to Take
Wash hands thoroughly before use. Gently pull down the lower eyelid to form a pocket. Apply a thin strip of ointment into the pocket. Close the eye gently for a few moments. Avoid touching the tube tip to the eye or any other surface to prevent contamination.
Mechanism of Action
Betamethasone is a potent corticosteroid that suppresses inflammation by inhibiting the release of inflammatory mediators. Neomycin is an aminoglycoside antibiotic that kills bacteria by irreversibly binding to the 30S ribosomal subunit, thereby inhibiting bacterial protein synthesis.
Pharmacokinetics
Onset
Anti-inflammatory and antimicrobial effects usually within hours to days.
Excretion
Betamethasone: renal. Neomycin: primarily renal if absorbed systemically.
Half life
Betamethasone: approximately 5-6 hours (systemic). Neomycin: not well-defined for topical ophthalmic use, systemic half-life is 2-3 hours if absorbed.
Absorption
Systemic absorption from ophthalmic application is generally minimal, but can occur, especially with prolonged use or damaged cornea. Betamethasone can be absorbed topically; Neomycin poorly absorbed through intact skin, but increased absorption through inflamed or damaged skin/mucosa.
Metabolism
Betamethasone: primarily hepatic. Neomycin: not significantly metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Betamethasone, Neomycin, or any component of the ointment.
- •Viral infections of the eye (e.g., herpes simplex keratitis, vaccinia, varicella).
- •Fungal infections of the eye.
- •Mycobacterial infections of the eye.
- •Untreated purulent eye infections.
Drug Interactions
Other topical corticosteroids
Concurrent use may increase systemic absorption and potential for side effects.
Other topical ophthalmic medications
If using other eye drops/ointments, wait at least 5-10 minutes between applications to avoid washing out.
Storage
Store below 30°C. Protect from light and moisture. Keep out of the reach of children.
Overdose
Accidental ingestion of the eye ointment is unlikely to cause systemic toxicity due to minimal absorption. Topical overdose may lead to increased local irritation or systemic corticosteroid effects with prolonged use. Discontinue use and consult a physician if overdose is suspected.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Avoid prolonged or extensive use. Exercise caution during lactation as small amounts may be excreted in breast milk. Consult a doctor.
