Overview of the medicine
Bethanechol is a cholinergic agent used to treat acute postoperative and postpartum non-obstructive urinary retention and for neurogenic atony of the urinary bladder.
Uses & Indications
Dosage
Adults
Oral: 10-50 mg three or four times daily. Dosage should be individualized to achieve the desired effect with the lowest possible dose. Subcutaneous: 2.5-5 mg, repeated hourly if necessary, up to four doses.
Elderly
Start with lower doses (e.g., 5-10 mg orally three times daily) and titrate carefully based on response and tolerability.
Renal_impairment
No specific dose adjustment guidelines are provided, but caution is advised due to renal excretion.
How to Take
Oral tablets should be taken on an empty stomach (one hour before or two hours after meals) to reduce the likelihood of nausea and vomiting. Subcutaneous injection should only be administered by trained healthcare professionals.
Mechanism of Action
Bethanechol directly stimulates muscarinic receptors of the parasympathetic nervous system, increasing the tone of the detrusor muscle of the urinary bladder, which leads to contraction of the bladder and urination. It also increases gastrointestinal motility.

Side Effects
Contraindications
- •Hypersensitivity to bethanechol.

- •Known or suspected mechanical obstruction of the gastrointestinal or urinary tract.
- •Pronounced bradycardia or hypotension, vasomotor instability, coronary artery disease, epilepsy, parkinsonism.
- •During convalescence from recent myocardial infarction or hypertension.
Drug Interactions
Anticholinergic agents
Antagonize the effects of bethanechol.
Cholinesterase inhibitors
Concurrent use may result in additive cholinergic effects and increased toxicity.
Ganglionic blocking agents
May cause a critical drop in blood pressure, usually preceded by severe abdominal symptoms.
Storage
Store at room temperature (20-25°C), away from moisture and direct light. Keep out of reach of children.
Overdose
Symptoms of overdose include muscarinic overstimulation (e.g., salivation, abdominal cramps, nausea, vomiting, sweating, flushing, bradycardia, hypotension, bronchospasm). Treatment involves symptomatic and supportive measures. Atropine is an antidote.
Pregnancy & Lactation
Pregnancy Category C. Bethanechol should be given to a pregnant woman only if clearly needed. It is not known whether bethanechol is excreted in human milk. Exercise caution when administered to a nursing mother.
