Description
Overview of the medicine
Betnozen-N Cream is a combination medicine used to treat inflammatory and infected skin conditions such as eczema and dermatitis, where bacterial infection is present or likely. It contains Betamethasone, a potent corticosteroid, and Neomycin, an aminoglycoside antibiotic.
Uses & Indications
Dosage
Adults
Apply a thin layer to the affected skin area 2-3 times daily, or as directed by a physician. Gently rub in.
Elderly
Same as adults; use with caution for prolonged periods or on large areas due to potential for increased systemic absorption.
Renal_impairment
Not typically a concern for topical application; however, in cases of extensive and prolonged use, systemic absorption may occur, requiring caution.
How to Take
For external use only. Apply a small amount of cream to the affected area and gently rub it in. Wash hands thoroughly before and after application. Avoid contact with eyes, mouth, and mucous membranes.
Mechanism of Action
Betamethasone acts as a potent corticosteroid, reducing inflammation, itching, and vasoconstriction. Neomycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis, leading to bactericidal action against susceptible bacteria.
Pharmacokinetics
Onset
Symptomatic relief usually begins within a few days of application.
Excretion
Absorbed betamethasone and its metabolites are excreted primarily via the kidneys. Unabsorbed neomycin is excreted unchanged in feces.
Half life
Systemic half-life of absorbed betamethasone is generally hours. For topical neomycin, systemic half-life is not clinically significant due to poor absorption.
Absorption
Minimal systemic absorption of both betamethasone and neomycin occurs through intact skin; absorption is increased with inflamed or damaged skin, prolonged use, and occlusive dressings.
Metabolism
Absorbed betamethasone is primarily metabolized in the liver. Neomycin is poorly absorbed and primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to betamethasone, neomycin, or any component of the cream
- •Untreated fungal, viral (e.g., herpes simplex, varicella), or parasitic infections of the skin
- •Tuberculosis of the skin
- •Acne vulgaris, rosacea, perioral dermatitis
- •Perforated eardrum (if applied near ear)
Drug Interactions
Systemic drugs
Systemic drug interactions are unlikely due to minimal topical absorption. However, with prolonged use over large areas, systemic absorption of betamethasone could theoretically interact with drugs affecting corticosteroid metabolism (e.g., CYP3A4 inhibitors).
Other topical preparations
Avoid concurrent use with other topical corticosteroids or antibiotics on the same area unless advised by a doctor, to prevent additive effects or interactions.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Acute overdose is unlikely with topical application. Prolonged or excessive use may lead to systemic effects of corticosteroids (e.g., Cushing's syndrome, adrenal suppression) or, very rarely, ototoxicity/nephrotoxicity from neomycin (if applied to large areas of broken skin or for prolonged periods). Symptomatic and supportive treatment should be given.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus, and under strict medical supervision. Avoid prolonged or extensive use. Exercise caution when administered to a nursing woman, as corticosteroids and neomycin may be excreted in human milk; however, systemic absorption from topical application is generally low.
