Description
Overview of the medicine
Betnozen-N Ointment is a combination medication containing a potent corticosteroid (Betamethasone Valerate) and an aminoglycoside antibiotic (Neomycin Sulphate). It is used to treat inflammatory skin conditions that are complicated by secondary bacterial infections.
Uses & Indications
Dosage
Adults
Apply a thin film to the affected skin area once or twice daily. Do not use for more than 7 days without medical supervision.
Elderly
No specific dosage adjustment needed, but caution advised due to thinner skin.
Renal_impairment
Minimal systemic absorption, so renal impairment is not typically a concern for topical use. However, prolonged use on large, broken skin areas might increase absorption, requiring caution.
How to Take
For external use only. Apply a thin layer to the affected area and gently rub in. Wash hands before and after application. Avoid contact with eyes, mouth, and other mucous membranes. Do not apply to large skin areas, broken skin, or use with occlusive dressings unless advised by a doctor.
Mechanism of Action
Betamethasone is a potent corticosteroid that provides anti-inflammatory, antipruritic, and vasoconstrictive effects. It acts by inducing phospholipase A2 inhibitory proteins, lipocortins, which control the biosynthesis of potent mediators of inflammation like prostaglandins and leukotrienes. Neomycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by irreversibly binding to the 30S ribosomal subunit, leading to bacterial cell death. This combination treats both inflammation and associated bacterial infection.
Pharmacokinetics
Onset
Anti-inflammatory effects typically within hours to days; antibacterial effects within days.
Excretion
Absorbed Betamethasone is excreted via kidneys. Absorbed Neomycin is excreted primarily unchanged via kidneys.
Half life
Not clinically relevant for topical application due to minimal systemic absorption.
Absorption
Minimal systemic absorption through intact skin; absorption increases with skin damage, occlusion, or application to large areas. Neomycin absorption is also minimal but can increase with inflamed skin.
Metabolism
Absorbed Betamethasone is metabolized primarily in the liver. Neomycin is poorly metabolized.
Side Effects
Contraindications
- •Hypersensitivity to betamethasone, neomycin, or any other component
- •Viral skin infections (e.g., herpes simplex, varicella)
- •Fungal skin infections
- •Tuberculosis of the skin
- •Acne vulgaris
- •Rosacea
- •Perioral dermatitis
- •Ulcerative conditions
- •Large or deep wounds
- •Perforated eardrum (if applied to ear)
Drug Interactions
Systemic corticosteroids
Increased risk of HPA axis suppression if used concurrently with other systemic corticosteroids.
Other ototoxic or nephrotoxic drugs
Increased risk of ototoxicity or nephrotoxicity if significant systemic absorption of neomycin occurs.
Storage
Store below 30°C in a cool, dry place, protected from light. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause acute systemic effects. However, prolonged or excessive use of corticosteroids can lead to Cushing's syndrome symptoms (e.g., thinning skin, striae, hyperglycemia). Excessive topical neomycin could theoretically lead to ototoxicity or nephrotoxicity if absorbed systemically, though this is rare. Treatment is symptomatic and supportive. Discontinue use if systemic effects occur.
Pregnancy & Lactation
Consult a doctor before use during pregnancy or lactation. Corticosteroids can be absorbed systemically and should be used with caution, especially during early pregnancy. Neomycin absorption is minimal, but risks to the fetus/infant are not fully established. Use only if potential benefit outweighs potential risk.
