Description
Overview of the medicine
Bevacizumab is a recombinant humanized monoclonal antibody that selectively binds to and neutralizes Vascular Endothelial Growth Factor (VEGF-A), thereby inhibiting angiogenesis and tumor growth.
Uses & Indications
Dosage
Adults
Typically 5-15 mg/kg intravenously every 2-3 weeks, depending on indication and concomitant therapy. Administered as an infusion over 30-90 minutes.
Elderly
No specific dosage adjustment required for elderly patients based on age alone.
Renal_impairment
No specific dosage adjustment recommended for patients with renal impairment.
How to Take
Administered as an intravenous infusion by a healthcare professional. Initial infusion over 90 minutes, subsequent infusions may be shortened to 60 or 30 minutes if well tolerated.
Mechanism of Action
Bevacizumab selectively binds to and inhibits the biological activity of human VEGF-A, preventing its interaction with VEGF receptors (Flt-1 and KDR) on the surface of endothelial cells. This inhibits new blood vessel formation (angiogenesis) that tumors need to grow.
Pharmacokinetics
Onset
Not immediately measurable in terms of clinical effect; takes several weeks for anti-angiogenic effects.
Excretion
Mainly via catabolism; not renally or hepatically cleared in a major way.
Half life
Approximately 20 days (range 11-50 days).
Absorption
Administered intravenously, hence 100% bioavailability.
Metabolism
Degraded via proteolytic catabolism like endogenous IgG.
Side Effects
Contraindications
- Known hypersensitivity to bevacizumab or any excipient
- Severe bleeding or recent significant hemorrhage
- Uncontrolled hypertension
- Recent major surgery or unhealed wounds
Drug Interactions
Sunitinib
Increased risk of microangiopathic hemolytic anemia with Bevacizumab.
Platinum-based and taxane chemotherapies
Increased risk of neutropenia and other myelosuppression, though specific interactions are not consistently demonstrated.
Storage
Store refrigerated at 2°C to 8°C (36°F to 46°F) in original carton to protect from light. Do not freeze. Do not shake.
Overdose
No specific antidote for overdose. Management should involve supportive measures and close monitoring for adverse reactions.
Pregnancy & Lactation
Pregnancy Category D. May cause fetal harm. Avoid during pregnancy. Advise women of reproductive potential to use effective contraception. Not recommended during breastfeeding due to potential for serious adverse reactions in the infant.
