Description
Overview of the medicine
Bicalutamide is an anti-androgen used in the treatment of prostate cancer. It is often prescribed in combination with other treatments like LHRH analogues or surgical castration.
Uses & Indications
Dosage
Adults
For advanced prostate cancer: 50 mg orally once daily. For locally advanced, non-metastatic prostate cancer: 150 mg orally once daily.
Elderly
No dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is considered necessary for patients with renal impairment.
How to Take
Administer orally once daily, with or without food. It should be taken at the same time each day.
Mechanism of Action
Bicalutamide is a non-steroidal anti-androgen that works by binding to androgen receptors in target tissues, blocking the effects of natural androgens (like testosterone and dihydrotestosterone) that stimulate prostate cancer cell growth. This leads to inhibition of tumor growth and progression.
Pharmacokinetics
Onset
Steady-state plasma concentrations of the active R-enantiomer are reached in about 10-14 days.
Excretion
Excreted approximately equally via the urine and feces as metabolites. Less than 2% is excreted unchanged.
Half life
The plasma elimination half-life of the active R-enantiomer is approximately 5.8 days.
Absorption
Well absorbed after oral administration. Peak plasma concentrations are achieved after approximately 13 days of daily dosing. Absolute bioavailability is not known.
Metabolism
Extensively metabolized in the liver by oxidation (to an inactive metabolite) and glucuronidation (primarily of the active R-enantiomer).
Side Effects
Contraindications
- •Hypersensitivity to Bicalutamide or any component of the formulation.
- •Not indicated for use in women or children.
Drug Interactions
Warfarin
Increased anticoagulant effect, monitor INR.
CYP3A4 inducers (e.g., rifampicin)
May decrease bicalutamide exposure.
CYP3A4 inhibitors (e.g., ketoconazole)
May increase bicalutamide exposure.
Storage
Store below 30°C, in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
There is no specific antidote for bicalutamide overdose. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Bicalutamide is contraindicated in women and should not be used during pregnancy or lactation. It can cause fetal harm.
