Description
Overview of the medicine
Bilanex 20 mg Tablet contains Bilastine, a second-generation H1-antihistamine primarily used for the symptomatic treatment of allergic rhinoconjunctivitis (seasonal and perennial) and urticaria (hives). It is known for its non-sedating profile at therapeutic doses.
Uses & Indications
Dosage
Adults
20 mg once daily.
Elderly
No dose adjustment is required in elderly patients.
Renal_impairment
No dose adjustment is required in patients with renal impairment.
How to Take
Bilanex 20 mg Tablet should be taken orally with water, on an empty stomach. It is recommended to take it one hour before or two hours after food or fruit juice (especially grapefruit juice).
Mechanism of Action
Bilastine acts as a selective peripheral H1-receptor antagonist. It prevents the binding of histamine to H1 receptors, thereby inhibiting the histamine-mediated effects responsible for allergic symptoms such as itching, sneezing, rhinorrhea, and skin reactions like urticaria.
Pharmacokinetics
Onset
Onset of action typically observed within 30-60 minutes.
Excretion
Approximately 67% of the administered dose is excreted unchanged in the feces and 28% in the urine, indicating primary elimination via the fecal route.
Half life
The elimination half-life is approximately 13 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached approximately 1.3 hours post-dose. Food, especially grapefruit juice, significantly reduces absorption.
Metabolism
Bilastine is minimally metabolized in humans; it does not undergo significant hepatic metabolism and is not a substrate for CYP450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to bilastine or any of the excipients.
Drug Interactions
Diltiazem
Increases plasma concentration of bilastine by about 50%.
Cyclosporine, Rifampicin
Other P-glycoprotein inhibitors/inducers may alter bilastine levels.
Ketoconazole and Erythromycin
May increase plasma concentration of bilastine due to P-glycoprotein inhibition.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
In cases of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote for bilastine. Clinical trials show that even at doses up to 80 mg once daily, adverse events were similar to placebo.
Pregnancy & Lactation
Use during pregnancy is not recommended unless the potential benefits outweigh the potential risks to the fetus. It is not known whether bilastine is excreted in human milk, so caution should be exercised if used during lactation.

Clinical Trials
Extensive clinical trials have demonstrated the efficacy and safety of bilastine for the treatment of allergic rhinoconjunctivitis and urticaria. Studies have shown comparable efficacy to other second-generation antihistamines with a favorable safety profile.
Lab Monitoring
- Routine laboratory monitoring is not generally required for patients taking bilastine.
Doctor Notes
- Bilastine is a preferred choice for patients requiring a non-sedating antihistamine.
- Emphasize strict adherence to administration instructions (empty stomach, avoidance of fruit juices) for optimal efficacy.
- Generally safe for elderly patients and those with renal impairment without dose adjustment.
Patient Guidelines
- Take Bilanex 20 mg Tablet on an empty stomach, at least one hour before or two hours after food or fruit juice.
- Do not exceed the recommended dose.
- Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to make up for a missed one.
Driving Precautions
Bilastine is generally considered non-sedating at therapeutic doses. However, individual reactions vary. Patients should be advised not to drive or operate machinery until they are aware of how they react to the medication.
Lifestyle Advice
- Avoid consuming fruit juices (especially grapefruit juice) while taking this medication as they can reduce its effectiveness.
- While bilastine is generally non-sedating, avoid alcohol as it can exacerbate potential sedative effects.
