Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It competitively inhibits the binding of histamine to H1 receptors, thereby preventing the effects of histamine such as itching, sneezing, and runny nose.
Pharmacokinetics
Onset
Onset of action is approximately 1 hour.
Excretion
Primarily excreted unchanged, with about 80% recovered in feces and 10% in urine.
Half life
The elimination half-life is approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are reached in approximately 1-3 hours.
Metabolism
Minimally metabolized in the liver (approximately 5-10% of the dose).
Side Effects
Contraindications
- Known hypersensitivity to fexofenadine or any of the excipients in the formulation.
Drug Interactions
Erythromycin and Ketoconazole
Can increase plasma concentrations of fexofenadine. No dosage adjustment is generally necessary, but monitor for increased side effects.
Antacids (containing aluminum or magnesium)
Concurrent use can reduce fexofenadine absorption. Separate administration by at least 2 hours.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Do not freeze.
Overdose
Symptoms of overdose include drowsiness, dizziness, and dry mouth. Management is symptomatic and supportive; there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted in breast milk; use with caution in nursing mothers.
