Description
Overview of the medicine
Bimatoprost is a synthetic prostamide analog with ocular hypotensive activity. It is used to lower intraocular pressure (IOP) in patients with open-angle glaucoma or ocular hypertension. It is also approved for the cosmetic treatment of hypotrichosis of the eyelashes to make them longer, thicker, and darker.
Uses & Indications
Dosage
Adults
One drop in the affected eye(s) once daily in the evening.
Elderly
No specific dose adjustment needed.
Renal_impairment
No specific dose adjustment needed due to minimal systemic absorption.
How to Take
For ophthalmic use only. Apply one drop in the conjunctival sac of the affected eye(s) once daily in the evening. If using other ophthalmic products, administer at least 5 minutes apart. Remove contact lenses before use and reinsert 15 minutes after.
Mechanism of Action
Bimatoprost is a selective prostamide F2α receptor agonist. It is believed to lower intraocular pressure (IOP) by increasing the outflow of aqueous humor primarily through the uveoscleral pathway and to a lesser extent, through the trabecular meshwork.
Pharmacokinetics
Onset
4 hours (for IOP reduction)
Excretion
Excreted primarily by the kidneys.
Half life
Approximately 45 minutes (systemically)
Absorption
Absorbed through the cornea. Peak plasma concentrations are achieved within 10 minutes post-dosing. Systemic exposure is very low.
Metabolism
Primarily metabolized via β-oxidation, N-deethylation, and amide hydrolysis.
Side Effects
Contraindications
- Hypersensitivity to bimatoprost or any excipient in the formulation.
Drug Interactions
Other prostaglandin analogs
Avoid concomitant use as it may reduce the IOP-lowering effect.
Storage
Store at room temperature (15°C to 25°C), protected from light. Do not freeze.
Overdose
Overdose is unlikely with ophthalmic administration. If overdose occurs, treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown whether bimatoprost is excreted in human milk; exercise caution when administered to a nursing mother.

Lab Monitoring
- Regular ophthalmic examinations including intraocular pressure (IOP) measurement.
Doctor Notes
- Counsel patients on the potential for irreversible iris pigmentation changes and proper administration technique.
- Monitor patients for signs of intraocular inflammation or macular edema, particularly in patients with a history of these conditions.
Patient Guidelines
- Use exactly as prescribed by your doctor.
- Do not allow the dropper tip to touch the eye or any other surface to avoid contamination.
- Remove contact lenses before applying and wait at least 15 minutes before reinserting.
- Be aware of potential irreversible changes in iris color and reversible changes in eyelid skin color and eyelash growth.
Missed Dose Advice
If a dose is missed, continue treatment with the next dose as scheduled. Do not double the dose to make up for a missed dose.
Driving Precautions
May cause temporary blurred vision immediately after instillation. Advise patients to wait until their vision clears before driving or operating machinery.
Lifestyle Advice
- Maintain regular follow-up appointments with your ophthalmologist to monitor IOP and eye health.
- Practice good eye hygiene.
