Olanzapine (Bizoran) is an atypical antipsychotic medication used to treat schizophrenia and bipolar disorder. It helps to restore the balance of certain natural substances (neurotransmitters) in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initially 10 mg once daily, target dose 10-15 mg/day. Bipolar mania: Initially 10-15 mg once daily (monotherapy) or 10 mg once daily (combination), target 5-20 mg/day.
Elderly
Lower initial dose (e.g., 5 mg/day) and slower titration may be considered, especially if risk factors for hypotension are present.
Renal_impairment
No specific dose adjustment is routinely required; however, caution is advised in severe renal impairment.
How to Take
Take Bizoran-5mg tablet orally, once daily, with or without food. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Olanzapine acts as an antagonist at multiple receptor sites, including dopamine D2, serotonin 5-HT2A, alpha1-adrenergic, histamine H1, and muscarinic M1 receptors. This broad receptor binding profile contributes to its antipsychotic and mood-stabilizing effects.

Well absorbed orally, reaching peak plasma concentrations in 5-8 hours. Bioavailability is not affected by food.

Contraindications
- Known hypersensitivity to olanzapine or any component of the formulation.
- Patients with narrow-angle glaucoma.
Drug Interactions
Fluvoxamine
May increase olanzapine plasma levels due to CYP1A2 inhibition.
Carbamazepine
May decrease olanzapine plasma levels due to enzyme induction.
Antihypertensives
May enhance hypotensive effects.
Alcohol/CNS Depressants
May have additive CNS depressant effects.

Symptoms of overdose include tachycardia, agitation, dysarthria, various extrapyramidal symptoms, and decreased consciousness (ranging from sedation to coma). Management is primarily supportive, including maintaining an open airway, oxygenation, and ventilation.
Pregnancy & Lactation
Pregnancy Category C (risk cannot be ruled out). Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding due to excretion in milk.
