Description
Overview of the medicine
Blast 20 mg Tablet is a proton pump inhibitor used to reduce stomach acid production, helping to treat conditions like gastric ulcers, GERD, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Gastric ulcer: 20 mg once daily for 4-8 weeks; GERD: 20-40 mg once daily for 4-8 weeks; Zollinger-Ellison syndrome: 60 mg once daily, adjusted as needed.
Elderly
No specific dose adjustment is generally required, but caution is advised due to potential reduced hepatic function.
Renal_impairment
No dose adjustment needed for renal impairment.
How to Take
Take orally, preferably in the morning before a meal. Swallow the tablet whole; do not chew or crush.
Mechanism of Action
Irreversibly binds to and inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, thereby blocking the final step of acid secretion.
Pharmacokinetics
Onset
1-2 hours
Excretion
Mainly excreted in urine (approximately 77%) and feces (approximately 18%).
Half life
0.5-1.5 hours
Absorption
Rapidly absorbed after oral administration; bioavailability is about 30-40% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to the active substance or any excipients
- Concomitant use with Nelfinavir
Drug Interactions
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Nelfinavir, Atazanavir
Reduced plasma levels of antiretrovirals.
Diazepam, Phenytoin, Citalopram
Increased plasma levels due to CYP inhibition.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms are generally mild and self-limiting (e.g., nausea, vomiting, dizziness, abdominal pain). No specific antidote; symptomatic and supportive treatment is recommended.
Pregnancy & Lactation
Use in pregnancy only if clearly needed and the benefit outweighs the risk. Small amounts are excreted in breast milk; caution should be exercised when administered to a nursing mother.
