Description
Overview of the medicine
Candesartan Cilexetil is an angiotensin II receptor blocker (ARB) used to treat high blood pressure and heart failure. It helps relax blood vessels, lowering blood pressure and improving blood flow.
Uses & Indications
Dosage
Adults
Initial dose 8 mg once daily; usual maintenance dose 8-32 mg once daily. For heart failure, initial dose 4 mg once daily, titrated up to 32 mg once daily.
Elderly
No initial dosage adjustment is necessary for elderly patients.
Renal_impairment
Initial dose 4 mg once daily in patients with severe renal impairment (creatinine clearance <30 mL/min/1.73 m²).
How to Take
Administer orally, once daily with or without food. Swallow the tablet whole with water.
Mechanism of Action
Candesartan selectively blocks the binding of angiotensin II to the AT1 receptor in various tissues, including vascular smooth muscle and the adrenal gland. This inhibits the vasoconstrictor and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased blood pressure, and reduced sodium and water reabsorption.
Pharmacokinetics
Onset
2-4 hours, full effect within 4 weeks.
Excretion
Primarily excreted unchanged in urine and bile.
Half life
Approximately 9 hours for candesartan.
Absorption
Rapidly absorbed from the GI tract after oral administration, converted to active candesartan. Bioavailability is approximately 15%.
Metabolism
Candesartan cilexetil is a prodrug that is completely metabolized to the active candesartan by ester hydrolysis in the GI tract during absorption. Candesartan itself is minimally metabolized by the liver.
Side Effects
Contraindications
- Hypersensitivity to candesartan cilexetil or any excipients.
- Pregnancy (especially second and third trimesters).
- Severe hepatic impairment and/or cholestasis.
- Concomitant use with aliskiren in patients with diabetes mellitus or renal impairment (GFR <60 mL/min/1.73 m²).
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Concomitant use with aliskiren in patients with diabetes or renal impairment is contraindicated due to increased risk of hypotension, hyperkalemia, and renal impairment.
NSAIDs (e.g., ibuprofen, naproxen)
May reduce the antihypertensive effect and increase the risk of renal impairment, especially in dehydrated or elderly patients.
Potassium-sparing diuretics (e.g., spironolactone), potassium supplements, salt substitutes containing potassium
Increased risk of hyperkalemia.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include hypotension and tachycardia; bradycardia could also occur. Treatment is supportive and symptomatic; lay the patient supine with legs elevated. If ingestion is recent, gastric lavage or induced emesis may be considered.
Pregnancy & Lactation
Discontinue if pregnancy is detected. Not recommended during breastfeeding as it is unknown if candesartan is excreted in human milk. Consult doctor.
