Description
Overview of the medicine
Flucloxacillin is a penicillin antibiotic used to treat a variety of bacterial infections, especially those caused by beta-lactamase producing bacteria. It is effective against infections of the skin, soft tissue, bone, and respiratory tract.
Uses & Indications
Dosage
Adults
250-500 mg orally every 6 hours, taken 30-60 minutes before meals. Severe infections may require higher doses.
Elderly
Similar to adult dosage, but dose reduction may be necessary in case of impaired renal function.
Renal_impairment
Dose adjustment required based on creatinine clearance. For severe impairment (CrCl <10 mL/min), reduce dose or extend dosing interval.
How to Take
Take orally with a glass of water, preferably on an empty stomach (at least 30-60 minutes before food or 2 hours after food) to ensure optimal absorption.
Mechanism of Action
Flucloxacillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Clinical effects usually begin within a few hours of administration.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. A small amount is excreted in bile.
Half life
Approximately 1 hour in adults with normal renal function.
Absorption
Well absorbed orally, best on an empty stomach. Peak plasma concentrations are reached within 30-60 minutes.
Metabolism
Partially metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Known hypersensitivity to flucloxacillin, other penicillins, or any component of the formulation.
- History of flucloxacillin-associated jaundice or hepatic dysfunction.
Drug Interactions
Warfarin
May alter INR (International Normalized Ratio), requiring close monitoring and dose adjustment of warfarin.
Probenecid
Increases and prolongs plasma levels of flucloxacillin by reducing renal tubular secretion.
Methotrexate
May increase methotrexate toxicity due to reduced renal clearance.
Oral Contraceptives
May reduce the efficacy of oral contraceptives (advise additional contraception methods).
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and electrolyte imbalance. In rare cases, convulsions have been reported. Management is largely supportive; hemodialysis may be considered in severe cases.
Pregnancy & Lactation
Pregnancy Category B. Flucloxacillin has not been shown to be teratogenic in animal studies, but should be used during pregnancy only if clearly needed. It is excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential adverse effects like diarrhea or candidiasis.

Lab Monitoring
- Liver function tests (LFTs) should be monitored during prolonged therapy or in patients with pre-existing hepatic impairment.
- Renal function should be monitored during prolonged high-dose therapy.
- Complete blood count (CBC) in prolonged treatment.
Doctor Notes
- Ensure patient has no history of penicillin allergy before prescribing.
- Advise patient to take on an empty stomach for optimal absorption.
- Monitor liver function, particularly in elderly or those with pre-existing hepatic conditions during extended therapy.
Patient Guidelines
- Complete the full course of antibiotics as prescribed by your doctor, even if symptoms improve.
- Take the medicine on an empty stomach for better absorption.
- Inform your doctor about any allergies, especially to penicillins or cephalosporins.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not double the dose to catch up.
