Overview of the medicine
Brexpiprazole is an atypical antipsychotic primarily used for the treatment of schizophrenia and as an adjunctive therapy for major depressive disorder. It helps improve mood, thinking, and behavior.
Uses & Indications
Dosage
Adults
For schizophrenia: Initial dose is 1 mg once daily, titrated to a target range of 2-4 mg/day. For MDD (adjunctive): Initial dose is 0.5 mg or 1 mg once daily, titrated to a target of 2 mg/day. The 0.5 mg tablet is often used for initiation and titration.
Elderly
No routine dose adjustment is required based solely on age, but consider age-related physiological changes and potential comorbidities. Use caution.
Renal_impairment
For moderate to severe renal impairment (CrCl < 60 mL/min), dose adjustment may be necessary. Max dose is 2 mg/day for schizophrenia and 1 mg/day for MDD.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Brexpiprazole acts as a partial agonist at serotonin 5-HT1A and dopamine D2 receptors, and as an antagonist at serotonin 5-HT2A and noradrenaline alpha2C receptors. This modulation helps balance neurotransmitter activity in the brain.

Onset
Clinical effects may take several weeks to become apparent.
Excretion
Mainly excreted in feces (about 60%) and to a lesser extent in urine (about 35%).
Half life
The elimination half-life is approximately 91 hours (for parent drug), and for the active metabolite, it's about 69 hours.
Absorption
Well absorbed after oral administration, with peak plasma concentrations occurring approximately 4 hours post-dose.
Metabolism
Primarily metabolized in the liver via CYP2D6 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Known hypersensitivity to brexpiprazole or any component of the formulation.
Drug Interactions
Strong CYP3A4 Inducers (e.g., rifampin, carbamazepine)
Increase brexpiprazole dose by approximately double.
Strong CYP3A4 Inhibitors (e.g., ketoconazole, clarithromycin)
Reduce brexpiprazole dose by approximately half.
Strong CYP2D6 Inhibitors (e.g., quinidine, fluoxetine, paroxetine)
Reduce brexpiprazole dose by approximately half.
Storage
Store at room temperature (20°C to 25°C), away from light and moisture. Keep out of reach of children.
Overdose
Management of overdose should include symptomatic and supportive treatment, including maintaining an adequate airway, oxygenation, and ventilation. Gastric lavage and activated charcoal may be considered. Closely monitor cardiovascular status and vital signs.
Pregnancy & Lactation
Pregnancy: Use only if the potential benefit justifies the potential risk to the fetus. Neonates exposed during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Lactation: Brexpiprazole is excreted in human milk. Advise patients to avoid breastfeeding during treatment.
