Brexpiprazole is an atypical antipsychotic medication used to treat schizophrenia and as an adjunctive treatment for major depressive disorder.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial 1 mg once daily, target 2-4 mg/day. Max 4 mg/day. MDD (adjunctive): Initial 0.5 mg once daily, target 1-3 mg/day. Max 3 mg/day.
Elderly
No specific dose adjustment recommended for elderly patients unless hepatic or renal impairment is present. Start with lower doses and titrate carefully.
Renal_impairment
For moderate to severe renal impairment (CrCl < 60 mL/min), maximum recommended dose is 3 mg/day for schizophrenia and 2 mg/day for MDD. No adjustment needed for mild impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole; do not crush, chew, or split.
Mechanism of Action
Brexpiprazole acts as a partial agonist at D2 and 5-HT1A receptors, and as an antagonist at 5-HT2A and noradrenergic alpha1B and alpha2C receptors. This unique receptor profile contributes to its therapeutic effects.

Onset
Gradual onset; full therapeutic effects may take several weeks.
Excretion
Excreted primarily through feces (60%) and urine (35%), mainly as metabolites.
Half life
Approximately 91 hours (long half-life).
Absorption
Well absorbed after oral administration. Peak plasma concentration (Tmax) is reached in 4-5 hours.
Metabolism
Extensively metabolized primarily by CYP2D6 and CYP3A4 enzymes.
Side Effects
Contraindications

Drug Interactions
Strong CYP3A4 Inducers (e.g., Rifampin)
Consider doubling brexpiprazole dose.
Strong CYP3A4 Inhibitors (e.g., Ketoconazole, Itraconazole)
Reduce brexpiprazole dose by half.
Strong CYP2D6 Inhibitors (e.g., Quinidine, Fluoxetine, Paroxetine)
Reduce brexpiprazole dose by half.
Storage
Store at controlled room temperature (20°C to 25°C), excursions permitted to 15°C to 30°C. Protect from moisture and light. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, tachycardia, and hypotension. There is no specific antidote. Treatment should be supportive, including airway management, cardiovascular monitoring, and activated charcoal administration.
Pregnancy & Lactation
Pregnancy: Use during pregnancy only if the potential benefit outweighs the potential risk to the fetus. Neonates exposed to antipsychotic drugs during the third trimester are at risk for extrapyramidal and/or withdrawal symptoms. Lactation: Brexpiprazole is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
